PFNA内固定手术治疗老年股骨粗隆间骨折47例疗效观察

摘　　要：目的探讨PFNA内固定手术治疗老年股骨粗隆间骨折的临床疗效。方法将该院94例老年股骨粗隆间骨折患者随机分为防旋股骨近端髓内钉(PFNA)内固定治疗组(观察组)和动力髋螺钉(DHS)治疗组(对照组),每组47例,观察两组患者的手术指标,并根据Harris髋关节功能评分标准对两组患者的临床疗效进行评价。结果观察组患者的手术时间、术中出血量及骨折愈合时间均少于对照组患者,组间差异有统计学意义(P<0.05)。观察组患者的疗效优良率为91.5%,显著高于对照组患者的74.5%,组间差异有统计学意义(P<0.05)。结论采用PFNA内固定手术治疗老年股骨粗隆间骨折临床疗效确切,是一种理想的治疗方式,值得临床进一步推广使用。Objective To investigate the clinical effect of PFNA internal fixation operation in the treatment of senile femoral intertrochanteric fracture. Methods 94 cases of senile femoral intertrochanteric fracture admitted in our hospital from January, 2010 to January, 2013 were randomly divided into proximal femoral nail anti-rotation（PFNA） internal fixation treatment group（observation group） and dynamic hip screw（DHS） treatment group（control group）, 47 cases in each group. The operation index of the cases in two groups was observed, and the Harris hip function score was used to evaluate the clinical effect of patients in two groups. Results The operation time, intraoperative blood loss, and fracture healing time of the observation group were less than those of the control group, there were statistically significant differences between two groups（P〈0.05）. The excellent and good rate of the clinical effect of observation group was 91.5%, significantly higher than the control group＇s 74.5%, there was statistically significant difference between two groups（P〈0.05）. Conclusion PFNA internal fixation operation is an ideal treatment for senile femoral intertrochanteric fracture with good clinical effect, and is worthy of further clinical application.
