髓内钉联合侧方钢板治疗非股骨峡部肥大型骨不连的临床研究

摘　　要：目的:研究髓内钉联合侧方钢板治疗非股骨峡部肥大型骨不连的临床疗效。方法:选取2010年8月-2013年1月笔者所在医院收治的30例非股骨峡部肥大型骨不连患者,根据骨不连手术方法不同分为给予髓内钉联合侧方钢板治疗的观察组(15例)和仅给予更换髓内钉治疗的对照组(15例),比较两组患者的骨不连愈合情况和关节功能情况。结果:观察组影像学愈合时间、完全负重下地时间为(135.1±19.1)d和(176.9±22.4)d,均低于对照组,差异有统计学意义(P<0.05)。观察组骨不连愈合率,术后半年、1年Harris评分,HSS评分均高于对照组,差异有统计学意义(P<0.05)。结论:髓内钉联合侧方钢板治疗有助于促进骨不连愈合、改善髋关节和膝关节功能,是治疗非股骨峡部肥大型骨不连的理想方式。Objective：To study the clinical efficacy of intramedullary nailing with lateral plate in the treatment of non-femoral isthmus hypertrophic nonunion. Method：Selection from August 2010 to January 2013,the author’s hospital treated 30 cases of femoral isthmus,fat big bone nonunion patients, according to different bone nonunion surgical method treatmenting with an intramedullary nail joint lateral plate were divided into the observation group（15 cases）, and only giving replacement of intramedullary nail treatment of the control group（15 cases）. Comparison of two groups of patients with bone nonunion healing and joint function.Result：Observation group radiographic healing time,full load and down time was （135.1＆#177;19.1） d and （176.9＆#177;22.4） d,lower than the control group,the differences were statistically significant（P＆lt;0.05）. Bone nonunion healing rate,half year after operation,1 year Harris score,HSS score were higher than the control group,the differences were statistically significant（P＆lt;0.05）.Conclusion：Intramedullary nailing with lateral plate help to promote healing and improve function of the hip and knee,is the ideal way of non-femoral isthmus hypertrophic nonunion treatment.
