颅脑损伤者智力及记忆障碍法医学鉴定的相关因素分析

摘　　要：目的探讨交通事故所致颅脑损伤者的智力和记忆障碍特点及相关影响因素。方法对近年受理的325例颅脑损伤者所做韦氏智力测验(IQ)及韦氏记忆测验(MQ)等级与影响因素进行分析。结果①325例脑外伤案例的智力测验评定等级:迟钝31例,边缘智能状态98例,轻度智能减退159例,中度智能减退30例,重度智能减退5例,极重度智能减退2例。智力障碍与脑损伤程度及范围有一定相关性。②325例伤者中MQ<70分有295例(90.77%),以短时记忆降低最为明显(F=93.98,P<0.01)。轻型脑损伤者多数智力水平虽在边缘智能状态以上,但仍普遍(78.30%)有记忆减退,伤后记忆减退人数多于智力减退人数,智力损伤与记忆损伤不平衡。③测验时机不同,部分测验结果不同,重复鉴定时心理测验影响因素增加。④心理测验综合评定结果与专家组鉴定结论比较,无显著差异(χ2=1.73,P>0.05),符合率为79.98%。结论韦氏智力测验(IQ)及记忆测验(MQ)等级受多种因素影响,评定时应进行综合评定。Objective To investigate the factors in patients with brain injury caused by traffic accident intelligence and memory barriers and related effects.Methods 325 patients with traumatic brain injury were analyzed with Wechsler Intelligence Scale for IQ and Wechsler Memory Scale MQ level.Results Of 325 patients with traumatic brain injury,there were 31 cases with clumsy,98 cases with the edge of intellectual status,159 cases with mild mental retardation,30cases with moderate mental retardation,5 cases with severe mental retardation and 2 cases with extremely severe mental retardation.There was correlation between intelligence and memory impairment and the degree of brain damage and range.There were 295 cases with MQ 〈 70.Most of patients with mild traumatic brain injury were in the edge of intellectual status with hypomnesia.The results of psychological test and identification of experts were not difference （x2 =1.73 P〉0.05）.Conclusion Intelligence test IQ and memory test MQ level is affected by many factors.The evaluation should be comprehensive evaluated.
