摘  要:目的研究脑出血行微创穿刺术病人的血肿CT值与首次抽吸血肿清除率的关系。方法回顾性分析95例脑出血行微创穿刺术病人的临床资料,测量血肿最大层面平均CT值及穿刺前后血肿面积,计算血肿清除率。统计分析平均CT值与血肿清除率的关系以及不同CT值对预后的影响。结果血肿清除率随平均CT值的增高呈下降趋势,二者呈负相关(r=-0.92,P〈0.01)。中CT值(64-80HU)病人比高CT值及低CT值病人预后好(P〈0.05)。结论应用微创穿刺术治疗脑出血。血肿CT值越低,首次抽吸血肿清除率越高。该术式对中CT值(64—80HU)的血肿效果好。Objective To study the relationship between CT value and clearance rate of hematoma after initial aspiration in mini-invasive craniopuncture for patients with cerebral hemorrhage. Methods Clinical data of 95 patients with cerebral hemorrhage were analyzed retrospectively, who underwent mini-invasive craniopuncture. Average CT value of the maximal transverse section and area of hematoma before and after craniopuncture were measured. The clearance rate of hematoma was calculated. The relationship between CT value and clearance rate was analyzed statistically. The influence of different CT values on prognosis was analyzed. Results The clearance rate of hematoma declined with the increasing of the average CT value, and the clearance rate of hematoma was negatively related to the average CT value (r = -0.92, P 〈 0.01). Patients with medium CT value (64 to 80 HU) had better outcome than other patients with high and low CT value (P 〈 0.05). Conclusions During the mini-invasive craniopuncture for patients with cerebral hemorrhage, the lower the average CT value of hematoma is, the higher the initial clearance rate will be. However, patients with medium,CT value (64 to 80 HU) have better prognosis.
