氟康唑片联合制霉菌素混悬液治疗糖尿病性念珠菌性包皮龟头炎的临床效果观察

摘　　要：目的观察氟康唑片联合制霉菌素混悬液治疗糖尿病性念珠菌性包皮龟头炎的临床效果。方法临床选取同时符合糖尿病和念珠菌性包皮龟头炎诊断标准的患者136例,随机分为治疗组、对照组,每组各68例。两组均在糖尿病规范化治疗的前提下给予氟康唑片口服治疗2周,同时治疗组外用制霉菌素混悬液清洗,自然晾干,5次/d;对照组外用纯净水清洗,方法同治疗组。根据皮疹消退、临床症状改善及念珠菌镜检结果判定和比较疗效。并观察治愈者停药2个月后的复发情况;观察不良反应发生情况。结果治疗组治愈64例,治愈率为94.12%,对照组治愈52例,治愈率为76.47%。两组比较差异有统计学意义(P<0.01)。治愈者停药2个月后治疗组复发5例,复发率为7.81%,对照组复发20例,复发率为38.46%,两组的复发率比较差异有统计学意义(P<0.01)。所有患者治疗期间均未出现明显的不良反应。结论氟康唑片联合制霉菌素混悬液治疗糖尿病性念珠菌性包皮龟头炎的近、远期疗效及降低复发率方面均较为理想,且安全可靠。Objective To observe the clinical effect of fluconazole tablets combined with nystatin suspension in the treatment of diabetic candidal balanoposthitis. Methods 136 patients diagnosed as diabetic candidal balanoposthitis were randomly divided into the treatment group and the control group,there were 68 cases in each group.Both groups were under the standard treatment of diabitics,and given fluconazole tablets orally for 2 weeks.In addition,the treatment group were treated with nystatin suspension washing at the same time,5 times a day and dry naturally while the control group with pure water cleaning,the treatment method was the same as the treatment group.The curative effect was evaluated and compared according to erythra subsiding,the improvement of clinical symptoms and microscopic test results of Monilia.The recurrence of cured patients after 2 months drug withdraw and the incidence of adverse reactions was observed. Results In the treatment group,64 cases were cured,the cure rate was 94.12%,while in control group,52 cases were cured,the cure rate 76.47% and there was a statistical difference between the two groups （P〈0.01）.There are 2 cases of palindromia in the treatment group after 2 months drug withdrawal,the recurrence rate was 7.81%,while 20 cases of control group relapsed,the relapse rate 38.46%,and there was a statistical difference between the two groups （P〈0.01）.No obvious adverse reactions were seen in all patients during treatment. Conclusion The short-term and long-term effect and reduction of the recurrence rate in the treatment of diabetic candidal balanoposthitis by flucona- zole tablets combined with nystatin suspension are more ideal,and it is safe and reliable.
