摘  要:目的探讨早期糖尿病肾病患者血糖、肾小球率过滤(GFR)、NAG酶与尿泛素核糖体结合蛋白52(UBA52)的关系。方法选择新疆医科大学第五附属医院住院患者101例,分为正常对照组(n=22)、糖尿病组(DM组,n=20)、糖尿病肾病组(DN组,n=59),分别测定血与尿UBA52、NAG酶、α1微球蛋白、血糖、糖化血红蛋白及肾小球滤过率(GFR),对3组各指标进行比较,并分析其与尿UBA52的关系。结果 (1)DN组NAG酶、α1微球蛋白、GFR、血糖、糖化血红蛋白及尿UBA52水平均高于正常对照组,差异有统计学意义(P<0.05);DM组NAG酶、α1微球蛋白、血糖及糖化血红蛋白与正常对照组相比差异均有统计学意义(P<0.05);DM和DN组比较,GFR在DN组中升高更为明显,差异有统计学意义(P<0.05),尿UBA52水平在DM和DN组中均有升高,尿UBA52在DN组中升高更明显,差异有统计学意义(P<0.05)。(2)NAG酶、糖化血红蛋白及肾小球滤过率对尿UBA52的升高有影响。结论早期糖尿病肾病患者血糖增高、肾小球率过滤增加、NAG酶增高可导致尿UBA52水平增加。Objective To study the relationship between the blood glucose, glomerular filtration rate, NAG enzyme with urinary UBA 52 in patients with early diabetic nephropathy. Methods 101 patients were divided into three groups: a control group of 22 healthy people, a group of 20 diabetics, and a group of 59 patients with diabetic nephropathy. UBA52 in urine and blood, NAG enzyme, α1 microglobulin, blood glucose, glycosylated hemoglobin, glomerular filtration rate and so on were measured. The data were compared and analyzed in terms of their correlation with UBA52. Results (1) There are significant differences between the control group and diabetic group and diabetic nephropathy group in index of NAG enzyme, α1- microglobulin, blood glucose, and glycosylated hemoglobin (P 〈0.05) ; there was a significant difference in hematuria UBA52 level between diabetic group and diabetic nephropathy group (P 〈0.05). (2) Partial correlation analysis showed that: NAG enzyme, glycosylatedhemoglobin, and glomerular filtration rate is related to elevated blood UBA52. Conclusion UBA 52 has greater clinical value for early diagnosis of diabetic nephropathy. The increase of blood glucose, the NAG enzyme, hemoglobin and glomerular filtration rate may induce the increase of UBA52 level.
