关节镜下PDS线张力固定前交叉韧带胫骨髁间嵴撕脱性骨折的疗效分析

摘　　要：目的 探讨关节镜下应用聚对二氧杂环已酮（PDS）线高张力固定前交叉韧带胫骨髁间嵴撕脱性骨折的临床疗效。方法 选择2008年10月-2013年1月在喀什地区第一人民医院骨科住院的胫骨髁间嵴撕脱性骨折患者42例，在关节镜下应用PDS线将撕脱的前交叉韧带止点“十”字交叉固定于髁间嵴附着点处，同时于胫骨中上段应用空心钉为PDS线提供张力，加强固定撕脱的骨折块。采用Lysholms—Tegner膝关节功能评分分析临床疗效。结果 42例随访时间3～12个月，平均6．5个月，Lysholms-Tegner评分为（96．78±1．33）分。Lachman试验均阴性，骨折愈合良好，无感染病例。结论 关节镜下应用PDS线结合胫骨中上段空心钉高张力固定前交叉韧带胫骨髁间嵴撕脱性骨折是一种创伤小、操作简单的手术，可牢固固定骨折块，愈合率高，术后膝关节功能恢复快，值得推广应用。Objective To explore the clinical effect of high tension PI）S suture fixation for anterior cruciate ligament avulsion fracture from tibial eminence by arthroscopy monitoring. Methods From October 2008 to January 2013. We used ＂cross＂ PDS suture to fixed anterior cruciate ligament avulsion from tibial eminence under the monitor of arthroscopy in our hospital, as well as application hollow nail to provide high tension for the PDS sutue, strengthened the fixed of the tear fracture. According to Meyors-Mcerer classification, the patients included 13 cases of type Ⅱ , 29 cases of type Ⅲ. The knee joint score were evaluated according to the lysholms standard before and after operation. Results Forty five cases were followed up for 3 to 12 months, with the average of 6.5 months, the postoperative knee function score was （96.78± 1.33） months after operation, the Lachman test were all negative, and the fractures were all healed, with no cases of infection. Conclusion Combined with the screw fixation in upper to middle tibia provided high tension of PDS suture to fixed avulsion fractures of anterior cruciate ligament from tibial eminence is a minimally invasive, simple arthroscopic operation. Which can firmly fixed fracture, with high healing rate, and the knee joint function recovered quickly after operation. Which prove to be an operation worthy of popularization and application.
