双能量冠脉CTA成像不同重建算法在高BMI患者中应用

摘  要:目的探讨双源CT双能量成像中Safire重建算法及FBP重建算法在高体质指数(BMI)患者冠状动脉检查中的应用,评价Safire算法改善图像质量的应用价值。方法收集50例临床怀疑冠心病患者的双源CT双能量扫描资料,最终35例纳入研究,体质指数为(30.06±3.22)kg/m2,通过重建得到常规FBP算法重建的100kV数据组、融合数据组(相当于120kV数据)和Safire算法重建的100kV数据组、融合数据组(相当于120kV数据),4组图像分别命名为FBP-A与FBP-B及Safire-A与Safire-B;由2名医师对35例共540个冠状动脉节段进行主观图像质量计分评价,应用Kappa检验分析其一致性;应用秩和检验对FBP组与Safire组相同能量间CNR与SNR对应比较及Safire组内两组能量的不同能量间CNR与SNR对应比较来进行客观图像质量的评价。结果主观图像质量评分结果具有良好一致性(Kappa值=0.801,P<0.05),客观图像质量比较FBP-A与Safire-A组间存在统计学差异(P<0.05);FBP-B与Safire-B组间存在统计学差异(P<0.05),Safire-A、B组CNR、SNR均优于FBP-A、B组,Safire-A与Safire-B组间无统计学差异(P>0.05);结论 Safire算法重建图像在双能量CT冠脉扫描中对高体质指数(BMI)的患者可明显降低图像噪声,提高图像质量。Objective To investigate the applications of two different types of reconstruction algorithm, Safire algorithm and FBP algorithm, for the dual-source CT dual-energy scanning in coronary examination for BMI patients. To evaluate the value of Satire algorithm to improve the image quality. Methods We analyzed data of dual-source CT dual-energy scanning of 50 cases with suspected coronary heart disease. Eventually 35 cases were included in the study. Body mass index BMI (30.06±3.22) 100 kV data from the group of conventional FBP algorithm were reconstructed by reconstruction as data fusion group (equivalent to 120 kV data) 100 kV data set and Safire algorithm reconstruction data, data fusion group (equivalent to 120 kV data). Four groups of images were named as FBP-A and FBP-B and Safire-A and Safire-B. Evaluation of subjective image quality scores of a total of 540 coronary segments from 35 patients were completed by two qualified doctors. The kappa tests were used for consistency analysis. The rank sum test used to compare between CNR and SNR corresponding groups with the same energy in two reconstruction algorithm groups, and for comparison between CNR and SNR different energy groups in Safire algorithm. Objective image quality evaluation was conducted through the above analysis. Results The results of subjective image quality score has a good consistency (Kappa=0.801, P 〈0.05), There was a statistically significant difference in objective image quality between FBP-A and Safire-A (P 〈0.05), and significant difference between the FBP-B and Safire-B group (P 〈0.05). CNR and SNR of Safire-A, B groups both are better than FBP-A, B groups. Objective image quality did not, significantly, differs between Safire-A and Safire-B (P 〉0.05). Conclusion Safire reconstruction algorithm in dual-energy CT coronary scanning can significantly reduce image noise and improve image quality for the patients with high body mass index (BMI).
