摘  要:目的以往研究表明,原发性高血压患者心率变异性明显降低,本文通过动态心电图检测技术,探讨心率变异性与原发性高血压的关系。方法选取66例原发性高血压患者为观察组,另选33例血压正常患者为对照组,对99例全部研究对象均行24小时跟踪的动态心电图检查,心率变异指标包括SDNN、RMSSD、SDSD、pNN50、SDNN index和SDANN,在高血压组和对照组2组患者之间比较上述心率变异指标。结果 SDNN在对照组为(122.56±31.58)ms,在原发性高血压组为(102.21±31.67)ms;RMSSD在对照组为(44.09±34.79)ms,在原发性高血压组为(39.58±17.42)ms;SDSD在对照组为(41.09±27.05)ms,在原发性高血压组为(33.53±16.80)ms;pNN50在对照组为(9.63±12.78)%,在原发性高血压组为(4.88±3.95)%;SDNN index在对照组为(42.15±23.01)ms,在原发性高血压组为(31.67±11.22)ms。原发性高血压组的心率变异性指标SDNN、RMSSD、pNN50、SDNN index较对照组明显降低,差异具有统计学意义(P<0.05),SDANN差异未见统计学意义。结论原发性高血压患者心率变异性明显降低,可能与迷走神经功能受损有关。24小时动态心电图是检测心脏自主神经张力的无创技术,用其来分析心率变异可用于评估原发性高血压患者是否有自主神经系统损伤及其损伤程度。Objective To explore the relationship between heart rate variability(HRV) and essential hypertension (EH) by dynamic ECG monitoring. Methods 66 cases of EH ( observation group) and 33 individuals with normal blood pressure( control group) were enrolled in this study. 24-h ECG(Holter) monitoring was performed in all subjects. HRV indexes such as SDNN, RMSSD, SDSD,pNN50, SDNN index and SDANN were compared between the two groups. Results In the control group, SDNN, RMSSD, SDSD, pNN50, SDNN index and SDANN was ( 122.56± 31.58 ) ms, ( 44.09 ± 34.79 ) ms,(41.09 ±27.05) ms, (14.86 ± 16.67)%, (42.15 ±23.01) ms and(85.18 ±50.36) ms,respectively;In the EH group,SDNN, RMSSD, SDSD, pNN50, SDNN index and SDANN was ( 102.21 ± 31.67 ) ms, ( 39.58 ± 17.42 ) ms, ( 33.53 ± 16.80) ms, (4.88 ± 3.95 ) %, ( 31.67 ± 11.22 ) ms and ( 74.92 ± 45.86 ) ms, respectively. The difference in SDNN, RMSSD, SDSD,pNN50 and SDNN index was statistical significant ( P 〈 0.05 ). Conclusion The HRV indexes (except SDANN) were significantly reduced in the EH patients which was possibly associated with the impaired vagus nerve. The 24-h ECG(Hoher) monitoring can be adopted to conduct the HRV analysis and evaluate the autonomic nerve tension impairment extent in EH patients.
