结扎速血管闭合系统在非脱垂阴式子宫切除术中的应用价值

摘　　要：目的探讨血管闭合钳一结扎速血管闭合系统（LigaSure）在非脱垂阴式子宫切除术中的应用优势及安全性。方法选取2009年10月～2012年12月在嘉兴市妇幼保健院住院行非脱垂阴式子宫切除术的患者90例，手术均由同一手术者操作完成，将其随机分为观察组和对照组，每组各45例。观察组采用LigaSure手术，对照组采用传统缝扎法手术。观察两组患者的手术时间、术中出血量、术后排气时间、术后下床活动时间、住院时间等；比较两组患者术后48h的舒适度、术后病率、术后抗生素应用时间及应用情况等。结果观察组手术时间为（63．71±17．65）min，术后排气时间为（19．06±3．57）h，术后下床活动时间为（24．06±5．73）h，住院时间为（5．06±0．41）d，均短于对照组【（75．94±19．11）min、（35．00±12．15）h、（35．72±5．81）h、（7．34±0．52）d】，观察组术中出血量为（56．61±21．73）mL，少于对照组[（111．88±57．78）mL]，差异有统计学意义（P〈0．05）。观察组患者术后48h舒适度（优良率为88．89％）明显优于对照组（优良率为77．78％），差异有统计学意义（P〈0．05）。观察组术后病率为13．33％，对照组术后病率为22．22％，两组比较，差异无统计学意义（P〉0．05）。观察组术后抗生素应用时间为（5．46±1．37）d，短于对照组[（10．62±2．04）d]，差异有高度统计学意义（P〈0．01）；观察组第三代头孢菌素使用率为33．33％，明显低于对照组（62．22％），差异有统计学意义（P〈0．05）。结论LigaSure应用非脱垂阴式子宫切除术中省时省力，可缩短手术时间，降低手术难度，减少术中出血量，缩短住院时间，提高患者的舒适度，减少抗生素的应用，安全可靠，值得临床推广。Objective To investigate the advantages and security of LigaSure in the treatment of non-prolapsed vaginal hysterectomy. Methods 90 cases of patients who were taken non-prolapsed vaginal hysterectomy in Maternal and Child Health Care Center of Jiaxing City, from October 2009 to December 2012, were selected. The surgical operations were completed by the same surgeon, and all patients were randomly divided into observation group and control group, 45 cases in each group. LigaSure was used in the observation group, traditional suture method was used in the control group. The operative time, blood loss, postoperative discharge time, postoperative ambulation time, hospitalization time of two groups was observed. Comfort level after 48 hours of patients, postoperative morbidity and postoperative antibiotics application time were compared between two groups. Results In the observation group, the operative time [（63.71± 17.65） min], postoperative discharge time [（19.06±3.57） h], postoperative ambulation time [（24.06±5.73） hi, hospitalization time [（5.06±0.41） d] were shorter than those of the control group [（75.94±19.11） min, （35.00±12.15） h, （35.72±5.81） h, （7.34±0.52） d]; blood loss of the observation group [（56.61±21.73） mL] was less than that of the control group [（111.88± 57.78） mL], the differences were statistically significant （P 〈 0.05）. Comfort after 48 hours of patients in observation group （88.89%） was better than that of control group （77.78%）, the difference was statistically significant （P 〈 0.05）. The postoperative morbidity of observation group （13.33%） was lower than that of the control group （22.22%）, but the difference was not statistically significant （P 〉 0.05）.
