髓外浆细胞瘤1例的临床与病理分析

摘　　要：本文报道一例髓外浆细胞瘤,患者,女性,55岁,以前额包块三个月入院,体检头颅前额部可见一8×7 cm2包块,血清免疫球蛋白量和骨髓浆细胞数正常,头颅MRI显示前额部梭形隆起,蝶窦、双则筛窦及上额窦可见异常信号,行鼻腔镜检及病理组织显示浆细胞瘤,免疫组化肿瘤细胞表达CD138、λ,通过复习文献讨论了此病的诊断标准和鉴别诊断,加深了对浆细胞瘤中的特殊类型髓外浆细胞瘤的认识和诊断水平。We reported a case of extramedullar plasmocytoma who was female and fifty-five years old. She was admitted to our wards due to 8×7 cm^2 mass in forehead. The number of serum immune globulin and the plasmocyte counts in marrow were normal. The head magnatic resonance imaging showed that there were fusiform upheaval in forehead and abnormal signal in sphenoid sinus, double frontal sinus, and ethmoidal sinus. The nasal cavity microscopic examination was carried out. The pathological tissue demonstrated plasmocytoma. The expression of CD138 and λwere position in immunohistochemical method. By means of reviewing literature, the diagnostic criteria and differential diagnosis of this disease were discussed. The level of understanding and diagnose for extramedullar plasmocytoma were improved.
