改良乳突切除术保留骨桥鼓室成形术治疗中耳乳突炎的体会

摘　　要：目的：研究改良乳突切除术保留骨桥鼓室成形术治疗中耳乳突炎治疗效果．方法：选取2013-01／2013-12来我院接受治疗的患有慢性化脓性中耳乳突炎的病患83例，对病患实行改良乳突切除保留骨桥同期鼓室成形术，首先对病患的耳部内侧进行切口，将外耳道的皮瓣和乳突进行暴露，之后使用电钻将乳突气房部分进行磨除，保留骨桥将乳突及鼓窦鼓室出现的病变组织进行清除．结果：病患听力得到改善的有效率为68．67％，治疗无效率为31．32％；病患在三个月内均获得干耳，慢性单纯型中耳乳突炎患者，有效率95．65％，骨疡型，有效率85．71％，最差为胆脂瘤型，有效率为34．37％．结论：对慢性化脓性中耳乳突炎病患尽早实行改良乳突切除保留骨桥鼓室成形术可以对病患的听力起到较好的治疗效果，值得在实际临床中进行推广．AIM： To investigate the clinical efficiency of the treatment for chronic otitis mastoidea after intactbridge mastoitym- panoplasty （IBM）. METHODS： The charts of 83 ears that suffered chronic otitis media （including patients with cholesteatoma, granulation type）, which underwent the treatment of IBM were reviewed. RESULTS： 83 cases were observed after 1 month to 4 years, and hearing improvement rate was 68.67% ; no efficiency rate was 31.32%, dry ear within 3 months. Hearing recovery efficiency of chronic otitis mastoidea is 95.65%, followed by chronic otitis media of 85. 71% and the worst, cholesteatoma, of 34. 37%. CONCLUSION： Long-term repeated pus hearing progressive declines in chronic suppurative otitis media patients should recieved modified mastoid bone bridge resection retaining undergoing tympanoplasty, and obtain dry ear, listening, to prevent hearing loss. On cholesteatoma, carious type otitis media patients, should remove the lesion as soon as possible based on the retention of mastoid air cells and the bone bridge simultaneous tympanoplasty.
