摘　　要：目的：分析和总结先天性心脏病（先心病）介入治疗严重并发症的原因和处理方法.方法：自1987年4月至2013年6月,我院小儿心脏科行先心病介入治疗患者2 715例.按年龄分为三组,＜3岁为婴幼儿组,3～6岁为学龄前组,6～18岁为学龄组.对介入治疗中、后发生的34例并发症的患者作回顾性分析.结果：①本组共34例严重并发症,发生率为1.26％.封堵器脱落、心律失常和溶血多见,均为7例,占总并发症20.5％;心包填塞2例,脑部并发症3例,瓣膜损伤2例,臂丛神经损伤3例,血管损伤2例,血小板减少1例.其中死亡4例.②婴幼儿组严重并发症最多16例,占47％,学龄前组12例,占35％.学龄组最少占18％,前两组各有2例死亡.结论：先心病介入治疗并发症需要充分认识,通过规范操作,努力减少并发症的发生,一旦发生能正确处理,严格随诊,使先心病介入治疗真正做到安全,有效.Objective：To analyze the incidence and cause of complications during interventional thera- py for congenital heart disease. Methods：Between April 1987 to June 2013, 2715 patients underwent the interventional therapy at the department of pediatric cardiology, Beijing Anzhen Hospital. The patients were devided into three groups： 〈3y group,3 - 6ygroup and 6 -18y group. Among them,34 patients with severe complications were retrospectively analyzed. Results： （1）The severe complications rate was 1.26 %. 34 patients with severe complications：of whom, occlude shift, arrhythmia and hemolysis 7cases and account for 20. 5% of the summery. Pericordis 2cases, cerebral complications 3cases, valve injury 2cases, Brachial plexus paralysis 3cases, vessel injury 2cases, Platlet decreasing lease. Death 4 cases, （2） in 〈 3 years group, severecomplications16cases, account for 47 % , 3-7 years group 12cases account for 35 %. 〉 7years group account for 18%, death happened in 〈 3years group and 3-7 years group. Conclusion： Transcatheter intervention of congenital heart diseases can be performed safely and successfully. The severe complications and mortality rate of interventional therapy are relative low. postoperative follow-up should be strictly administered.
