家庭医生制模式下“送康复服务上门”效果评价

摘　　要：目的评价家庭医生制模式下"送康复服务上门"为社区残疾人群的康复、完善"送康复服务上门"服务提供依据。方法以社区残疾人群为研究对象,按照提供服务的主体不同分为康复医生组、普通全科医生组和医生助理组,采用自身对照干预研究方法,分析比较社区残疾人群接受"送康复服务上门"服务前后的自测健康评定量表(SRHMS)得分和研究对象接受服务的满意度以及家庭医生的康复知识背景对"送康复服务上门"效果的影响。结果本次研究对象共207人,在接受家庭医生或医生助理"送康复服务上门"服务6个月后,SRHMS的总得分和生理、心理、社会健康三个子量表得分均有显著提高,差异有统计学意义(p<0.01);同时家庭医生的康复知识背景对"送康复服务上门"效果有差别,生理健康得分差异有统计学意义(p<0.05)。结论家庭医生团队的"送康复服务上门"服务模式能有效提高残疾人的生活质量,受到社区残疾人群的欢迎,但家庭医生康复专业知识不足,影响了"送康复服务上门"的效果,急需培训和提高。Objective To evaluate the effectiveness of outreach rehabilitation service on the community disabled residents by general practitioner. Method The scores the self- rated health measurement scale （SRHMS） were rated among 207 community disabled residents before and after the delivery of outreach rehabilitation service. The social and economic benefits and the subjects＇ satisfaction of the outreach rehabilitation service were measured. Result The total SRHMS score and the subacale scores of physiological health, mental health, and social health significantly increased six months after the delivery of outreach rehabilitation service （P 〈0. 01 ）. The subjects＇ satisfaction towards the service as a whole and of each service related item （content, mode, attitude, quality, frequency, and effect） ranged 71. 50% - 94. 69%. Conclusion The outreach rehabilitation service provided by GP could effectively improve the quality of life of disabled residents in the community and GP＇s professional training of rehabilitation should be strengthened.
