妊娠期糖尿病孕妇孕晚期血脂水平的变化及其与新生儿出生体重的相关性研究

摘　　要：目的研究妊娠期糖尿病孕妇孕晚期血脂水平的变化及其与新生儿出生体重的相关性。方法选择正常糖耐量孕妇(对照组)和妊娠期糖尿病孕妇(妊娠期糖尿病组)各52例,采用日立7080型全自动生化分析仪测定血清总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)和脂蛋白(a)[LP(a)],同时记录两组孕妇的年龄、孕周、妊娠前体重与新生儿出生体重等。结果 GDM组TG、LDL-C均显著高于对照组,差异有统计学意义(P<0.05)。TG与新生儿出生体重呈显著正相关(P<0.05),LP(a)与新生儿出生体重呈显著负相关(P<0.05)。结论妊娠期糖尿病孕妇存在以TG、LDL-C升高为特点的高脂血症,与正常糖耐量孕妇相比,伴发了更为严重的血脂代谢异常,并明显影响着新生儿的出生体重,因此对妊娠期糖尿病孕妇进行孕晚期血脂水平的监测和必要的饮食指导具有重要的临床意义。Objective：It is to investigate the changes of lipid levels in GDM and the relationship of lipid with neonatal birth weight. Methods：To select the NGT pregnant women（control group）and pregnant women with gestational diabetes mellitus（gestational diabetes group）with 52 cases in each,Hitachi 7080 automatic biochemical analyzer was used to measure the serum total cholesterol（TC）,triglyceride（TG）,high density lipoprotein cholesterol（HDL-C）,low density lipoprotein cholesterol（LDL-C）and lipoprotein（a）[LP（a）.At the same time,maternal age,gestational age,body weight before pregnancy and fetal birth weight were recorded in two groups.Results：The levels of TG and LDL-C were significantly higher in GDM group than those in the NGT group（P0.01）. The TG was postively correlated with neonatal birth weight（P0.05）.The LP（a）was negatively correlated with neonatal birth weight（P0.05）. Conclusion：Hyperlipidemia in pregnant women with gestational diabetes mellitus,with TG,LDL-C is elevated. Gestational diabetes in pregnant women with normal glucose tolerance compared,with more serious abnormal lipid metabolism,and significantly affect neonatal birth weight. Therefore,it has important clinical significance of monitoring serum lipid levels in pregnancy and the necessary dietary guidance for women with gestational diabetes.
