发生过临时性行为的中老年男性的艾滋病相关知识和行为研究

摘　　要：目的了解发生过临时性行为的中老年男性的社会背景,艾滋病相关知识、意识及行为状况、临时性行为特征、艾滋病病毒(HIV)和梅毒的感染状况。方法在浙江省某市的8个乡镇(街道)中,选取承认在一年内有过临时性行为的247名50岁及以上男性进行一对一的访谈。结果发生过临时性行为的中老年男性中,90.69%承认与临时性伴存在商业关系(金钱关系),9.31%否认存在商业关系。该人群文化程度低,非婚比例高,家庭幸福感低,与配偶和固定性伴的性生活满意度低,自我保护意识低,艾滋病的宣传干预和检测率低。与临时性伴发生性关系时安全套使用率低,但非农职业(民企、机关事业单位上班或经商者)者比在家务农者安全套使用率高(χ2=15.580,P<0.001);对固定性伴性生活满意度高者临时性生活频率较低(χ2=8.652,P=0.013)。HIV抗体阳性率为0,梅毒检测阳性率为1.62%。梅毒检测阳性者比阴性者较倾向于选择来自非固定场所的临时性伴(P=0.010),临时性生活频率也较高(P<0.0001)。结论发生过临时性行为的中老年男性来自社会各个阶层,急需重视他们的艾滋病的检测、干预工作。Objective To understand the social background, AIDS-related knowledge and behaviors, the characteristics of temporary sexual behaviors, and the infection rates of HIV and syphilis in middle-aged and elderly men having temporary sexual behaviors. Methods The study enrolled 247 men over 50 years old having a temporary sexual behavior within a year from 8 towns or streets in a city in Zhejiang province, and their information were collected through one to one interviews. Results Of the subjects enrolled, 90.69% had commercial sexual behavior, and 9.31% denied having commercial relationships. They had low level of education, high proportion of non-marital, low family happiness, low sexual satisfaction with their spouses and regular partners, low self-protection awareness, and low rate of AIDS intervention and detection. The rate of condom use in temporary sex was low, but it was higher in non-farmers （such as staff of private enterprises, government departments and institutions, or merchants） than that in farmers （x^2 = 15. 580, P〈0. 001）. The subjects satisfied with their spouses or regular partners had a lower frequency of temporary sex （ x^2 =8. 652, P=0. 013）. The positive rate of HIV was 0, and the rate of syphilis was 1.62%. Compared with the negatives for syphilis, the positives tended to choose temporary sex partners from nonfixed places （P=0. 010）, and they had a higher frequency of temporary sex （P〈0. 0001）. Conclusion Middle-aged and elderly men with temporary sexual behaviors come form varied social classes. We must pay more atten- tion to their HIV testing and interventions.
