高血压合并阻塞性睡眠呼吸暂停低通气综合征患者持续气道正压通气治疗前后血压与血管紧张素水平的研究  

The Effect of CPAP Short-term Treatment on Blood Pressure and Angiotensin Level in Refractory Hypertension Patients Complicated with OSAHS

摘　　要：[目的]观察持续气道正压通气(continuous positive airway pressure,CPAP)对治疗高血压合并阻塞性睡眠呼吸暂停低通气综合征(obstructive sleep apnea-hypopnea syndrome,OSAHS)患者的血压、血浆醛固酮(aldosterone,Aldo)、肾素活性(plasma renin activity,PRA)以及血管紧张素II(angiotensin II,AngII)水平的影响。[方法]入选高血压合并OSAHS的患者40例,测量晨起(7:00)、中午(12:00)、傍晚(17:30)、睡前(22:00)4个时间点血压,并抽血检测其血浆PRA、AngII及Aldo水平。患者连续4晚使用CPAP治疗,再次测量患者血压和血浆PRA、AngII及Aldo水平,比较治疗前后血压及PRA、AngII、Aldo的变化。[结果]治疗4d后患者血压均较治疗前有显著下降(P<0.01),血浆PRA、AngII较前增加(P<0.01);Aldo水平较前明显下降(P<0.01)。[结论]CPAP治疗降低了高血压合并OSAHS患者的血压,并能有效的改善血管紧张素水平。[Objective]To investigate the effect of continuous positive airway pressure（CPAP） short-term treatment on blood pressure and angiotensin level in refractory hypertension patients complicated with moderate or severe obstructive sleep apnea-hypopnea syndrome（OSAHS）.[Methods]40 patients with refractory hypertension complicated with moderate or severe OSAHS were recruited.The patients＇blood pressure were measured at 7AM,12AM,5：30PM and 10PM respectively.And the levels of blood levels of plasma renin activity（PRA）,angiotesnin Ⅱ （Ang Ⅱ）,aldosterone（Aldo） were determined withradioimmunoassay.All patients received 4 days of nCPAP treatment.Aiter 4 days,the pauents＇blood pressure was measured and the levels of plasma renin activity（PRA）,angiotesnin Ⅱ （Ang Ⅱ）,aldosterone（Aldo） were tested again at the same time points.The difference between blood pressure and angiotensin levels was analyzed compared with treatment.[Results] 4 days of nCPAP treatment lowered the level of angiotensin and decreased the SBP,DBP,MABP at 7AM and 5：30PM and MABP at 10PM and MSBP,MDBP,MPP,MABP at all time points significandy in the patients of refractory hypertensioncom plicated with OSAHS.[Conclusion] Short-term nCPAP treatment can decrease the level of angiotensin and the blood pressure in refractory hypertension patients complicated with moderate or severe OSAHS.
