摘　　要：目的探讨依托伤口造口小组的皮肤护理管理模式并评价管理的效果。方法成立伤口造口小组,确定三级管理体制的皮肤管理运行模式,明确各级人员职责,完善专科管理制度和流程,对相关人员进行培训,实行专科护理会诊制度,重视皮肤并发症的预防及治疗工作,对全院皮肤护理情况进行全程监控。结果皮肤护理管理模式建立前后1年比较压疮上报率达100%,高危压疮患者压疮发生率降低4.3%,压疮治疗有效率提升了18.52%;压疮治疗及伤口相关知识掌握情况比较,差异有统计学意义(均P<0.05)。结论伤口造口小组的皮肤管理模式的建立与运行显著拓展了护理工作内涵,使护理人员的造口伤口护理知识大幅提高,认为建立造口伤口小组,从根本上提升了全院皮肤管理的护理质量,是可行、有效的专科护理管理模式。Objective To discuss the establishment of skin management mode in wound stoma creation team, and its effect after operation. Methods Wound stoma creation team was established to determine the 3-level skin management mode. Personnel duty of each level was clarified, and the specific management system and the procedures were perfected. The relevant staff was trained, and consultation system of specific nursing was applied. Prevention and treatment of complications of the skin was paid great attention to, and the skin nursing situation was fully monitored around all the hospital. Results Compared the situation within 1 year before and after skin management system establishment, reported rate of pressure scores reached 100%, pres-sure scores incident was reduced by 4. 36% in patients with high-risk of pressure scores, and the efficiency rate of the treatment was increased by 18. 52%;Compared the understanding about the pressure scores treatment and wound, the difference was statistically significant （P〈0. 05）. Conclusion Establishment and application of the skin management mode in wound stoma creation team can significantly expand the connotation of nursing work, thus greatly increasing the nursing knowledge of nursing staff on wound stoma. It is believed that the establishment of wound stoma team can improve the nursing quality of skin management of the whole hospital fundamentally, which is a feasible and effective nursing management mode.
