摘　　要：目的研究青藤碱联合甲氨蝶呤对类风湿关节炎(RA)患者体内IL-1,IL-17和TNF-α表达的影响。方法 RA患者60例随机分为2组:对照组30例给予甲氨蝶呤治疗,治疗组30例给予甲氨蝶呤联合青藤碱治疗,观察2组治疗前后的体征和相关临床指标(如DAS28、ESR、RF、CRP)变化;采用qRT-PCR法检测治疗前后IL-1,IL-17和TNF-αmRNA的表达情况。结果治疗后2组患者的体征、相关临床指标均有明显改善,治疗后治疗组外周血淋巴细胞中IL-1,IL-17和TNF-αmRNA的表达水平较治疗前及对照组更低。结论青藤碱联合甲氨蝶呤治疗可通过调节外周血淋巴细胞的免疫功能而对RA患者起到显著治疗效果。Objective It is to investigate the effect of sinomenine combined with methotrexate on the expression of IL-1,IL-17 and TNF-α in rheumatoid arthritis （RA） patients.Methods 60 patients with RA were randomly divided into 2 groups：the control group （n =30） was treated with methotrexate,and the treatment group （n =30） was treated with sinomenine combined with methotrexate.The changes of signs and correlated clinical indexes such as DAS28,ESR,RF and CRP before and after treatment were observed in both groups,the expression of IL-1,IL-17 and TNF-α mRNA before and after treatment were determined by qRT-PCR.Results After treatment the signs and correlated clinical indexes in the patients were all improved significantly in both groups,and the expression levels of IL-1,IL-17 and TNF-α mRNA in lymphocytes in peripheral blood were significantly lower than those before treatment and in control group.Conclusion Sinomenine combined with methotrexate therapy has a significant curative effect on RA by regulating the immune function of peripheral blood lymphocytes.
