地佐辛用于小儿腹部手术术后静脉镇痛的效果及对生命体征的影响

摘　　要：目的观察地佐辛用于小儿腹部术后静脉镇痛的效果及其安全性,探讨其合适用量。方法选择90例行腹部手术的患儿,随机分为3组,每组30例,分别予地佐辛0.6mg/kg+昂丹司琼0.16 mg/kg(A组),地佐辛0.8 mg/kg+昂丹司琼0.16 mg/kg(B组)及芬太尼16μg/kg+昂丹司琼0.16 mg/kg(C组)术后持续静脉泵入以镇痛,比较各组术后4 h、12 h、24 h和48 h生命体征、FPS-R评分、CHEOPS评分、镇痛泵给药键按压次数以及用药后不良反应。结果术后3组不同时点的HR、RR、MAP及Sp(O2)均在正常范围内,3组间各指标比较差异无统计学意义。不同时点FPS-R评分、CHEOPS评分C组高于A组,A组高于B组。48 h内平均按压给药键次数C组高于A组,A组高于B组。总不良反应发生率A组与B组比较差异无统计学意义,B组明显低于C组。结论 0.8 mg/kg剂量地佐辛用于小儿腹部手术后静脉自控镇痛效果好,生命体征平稳,不良反应少。Objective It is to observe the effect and safety of dezocine on postoperative intravenous analgesia in children after abdomen surgery,and to explore the right dosage.Methods 90 children undergoing abdomen surgery were randomly divided into 3 groups with 30 cases in each group which were given different postoperative intravenous analgesia：dezocine 0.6 mg/kg ＋ ondansetron 0.16 mg/kg（group A）,dezocine 0.8 mg/kg ＋ ondansetron 0.16 mg/kg（groupB）,fentanyl 16 μg/kg ＋ ondansetron 0.16 mg/kg（group C）.The vital signs,FPS-R,CHEOPS,pressing times at 4 h,12 h,24 h,48 h after operation and adverse effect in three groups were compared.Results The HR,RR,MAP and Sp（O2） were all within normal limits at 4h,12 h,24 h,48 h after operation,these index had no significant difference between 3 groups.At different time,FPS-R scores and CHEOPS scores in group C were higher than those in group A,and the FPS-R scores and CHEOPS scores in group A were higher than those in group B.The pressing times within 48h in group C were more than that in group A,and the times in group A were more than that in group B,the differences were all significant.There was no significant difference in the total side effect rate between group A and group B,but the rate in group B was obviously lower than that in group C （P 〈 0.05）.Conclusion For postoperative intravenous analgesia in children with operation on abdomen,the dose of 0.8 mg/kg dezocine＇s effect is good,which can make the vital signs stable and decrease adverse reaction.
