组织工程胶原海绵膜真皮替代物修复创面的临床观察

摘　　要：目的:对采用组织工程胶原海绵膜真皮替代物修复创面的临床效果进行观察和分析,探讨临床进行创面修复的最佳方法。方法:选取2010年10月-2012年10月笔者所在医院收治的出现高发褥疮的24例危重患者,按照住院顺序以及创面类型将患者随机分为观察组和对照组,各12例。对照组采用常规凡士林纱布覆盖创面,观察组采用组织工程胶原海绵膜覆盖创面,所有患者的包扎和固定均采用常规手法,观察并比较两组患者的临床治疗效果。结果:观察组患者完全愈合时间明显短于对照组,愈合质量优于对照组,差异有统计学意义(P<0.05)。两组患者心脏、肾脏、肝脏还有肺脏等重要器官的功能均表现良好,无显著差异。对照组有1例(8.33%)患者出现原创面挛缩,7例(58.33%)患者出现明显的瘢痕增生;观察组无一例出现不良反应。结论:组织工程胶原海面膜作为一种新型真皮替代物应用于患者的创面修复中,疗效确切,具有安全性和良好生物兼容性,机体无任何排异反应,在神经外科危重患者高发褥疮的临床治疗中具有广阔的发展前景。Objective：To observe and analyze the clinical effect of using tissue engineering collagen dermal substitute to repair the wound, investigating the best method for clinical wound repair.Method：From October 2010 to October 2012 in our hospital 24 cases of critically ill patients withhigh-risk bedsore, in accordance with the hospitalization order and wound type patients were randomly divided into observation group and control group, 12 cases in each.The control group were treated with vaseline gauze to cover the wound, the observation group were treated with tissue engineering collagen membrane to cover the wound, all wrapped and fixed in patients using conventional techniques, clinical treatment effects were compared between the two groups of patients.Result：Observation group of patients with complete healing time is shorter than the control group, the healing quality is better than that of control group, the difference was statistically significant （P〈0.05）. Two groups of patients with critical organs such as heart, kidney, liver and lung function were performed well, there was no significant difference. Control group with 1 case （8.33%） patients experienced the original wound contracture, 7 cases （58.33%） patients experienced obvious scar hyperplasia; Observation group no adverse reaction.Conclusion：Tissue engineering collagen sea mask as a new dermal substitutes used in wound repair in patients, efficacy, safety and good biological compatibility, no rejection of the body, it has a broad development prospect in clinical treatment in critically ill patients with bedsore in Department of neurosurgery.
