卵巢间质血流在预测高龄不孕妇女卵巢储备中的应用

摘　　要：目的在体外受精-胚胎移植(in-vitro fertilization,IVF-ET)过程中监测卵巢间质血流的阻力指数(resistance index,RI)、收缩期峰流速(stromal peak systolic velocity,PSV)的变化,探讨其与基础卵泡刺激素(basal follicle stimulating hormone,bFSH)正常的高龄不孕妇女卵巢储备功能的关系。方法选取在本生殖中心行体外受精-胚胎移植(IVF-ET)或卵胞浆内单精子注射(intracytoplasmic sperm injection,ICSI)治疗的46例bFSH正常的高龄不孕症妇女,测量控制性超促排卵(COH)周期用药第l天(D1)卵巢间质血流的RI、PSV,结合阴道彩超监测基础窦卵泡数(antral follicle count,AFC)。根据获卵总数将病人分成反应不良组和反应正常组,比较两组间卵巢间质血流的RI、PSV、卵巢基础窦卵泡数的差异。结果 RI、PSV、AFC在卵巢储备功能不同的高龄不孕妇女中差异有统计学意义(P<0.05)。结论卵巢间质血流RI、PSV、卵巢基础窦卵泡数与bFSH正常的高龄不孕妇女卵巢储备功能有密切关系,是有效预测卵巢储备功能的指标。Objective To investigate the relationship between the ovarian stromal blood tlow index （RI, PSV） and ovarian reserve function of older infertile women whose basal follicle stimulating hormone was in regular. Methods 46 elder infertile women, received in-vitro fertilization or intra-cytoplasmic sperm injection in our hospital, were divided into two groups according to the number of oocytes collected. RI, PSV of ovary stromal blood flow and AFC were examined on the first day of COH（DI） , and compared RI,PSV and AFC between the two groups. Results RI, PSV and AFC were significantly different between the two groups. Conclusions RI,PSV and AFC are valuable in 13redicting ovarian functJion of elder infertile women.
