胺碘酮长期联合美托洛尔治疗心房纤颤的临床效果观察及安全性评价

摘　　要：目的观察胺碘酮长期联合美托洛尔治疗心房纤颤的临床疗效和安全性,评价其临床应用效果,为今后临床治疗提供参考和借鉴。方法随机选取我院2010年9月至2012年9月期间收治的心房纤颤患者98例,采用随机数字法将其分为对照组和观察组,对照组给予美托洛尔治疗,观察组则给予胺碘酮长期联合美托洛尔治疗,对比分析疗效,评价用药安全性。结果观察组的总有效率为89.80%明显高于对照组的63.27%(χ2=20.275 3,P=0.000 0),观察组左心室射血分数及左房内径的改善程度明显优于对照组(P<0.05),均未出现严重的不良反应。结论胺碘酮长期联合美托洛尔治疗心房纤颤能有效地恢复患者的左心室射血分数及左房内径,而且小剂量的胺碘酮长期联用美托洛尔治疗心房纤颤也相对安全,疗效显著,值得在临床实践中广泛的应用和推广。Objective To observe the clinical effect and safety on long-term treatment with amiodarone combined with metoprolol for atrial fibrillation, and to evaluate the clinical effect, to provide reference for further clinical treatment. Methods In this study, we randomly selected 98 patients with atrial fibrillation who were treated in our hospital from September 2010 to September 2012, and divided them into the Control Group and the Observation Group by the random number method. The Control Group received treatment with metoprolol, while the Observation Group was given long-term treatment with amiodarone combined with metoprolol. The primary outcome was a comparative analysis of the clinical effect of the both of groups and an evaluation of the drug safety. Results The total efficiency of the Observation Group was 89.80%, significantly higher than the 63.27%of the Control Group （χ^2=20.275 3, P=0.000 0）, the improvement of the left ventricular ejection fraction and left atrial diameter in the Observation Group was significantly better than those of the Control Group （P 〈0.05）, and no serious adverse reactions occurred on both of groups. Conclusions Long-term amiodarone treatment combined with metoprolol can effectively recover the left ventricular ejection fraction and left atrial diameter in patients with atrial fibrillation. Long-term treatment with small doses of amiodarone combined with metoprolol is relatively safe for atrial fibrillation, with a significant effect. It is worthy of widely applying in clinical practice and promoting.
