角膜绷带镜提高翼状胬肉患者术后舒适度的效果观察

摘　　要：目的探讨翼状胬肉患者术后佩戴角膜绷带镜对舒适度的改善情况。方法将100例行翼状胬肉切除联合角膜缘干细胞移植的单眼原发性翼状胬肉患者,按随机数字表分为对照组和观察组各50例,对照组给予常规护理,观察组在常规护理基础上应用角膜绷带镜,对患者术后眼部不适症状进行问卷量表评分,比较眼部疼痛程度,观察角膜创面愈合情况。结果观察组患者的刺激感、烧灼感、眼部刺痛、流泪、睁眼困难等不舒适症状评分均低于对照组,差异具有统计学意义(P<0.001);观察组患者术后眼部疼痛程度较轻,差异有统计学意义(P<0.05);观察组术后2 d内角膜创面愈合者32例(64%),3 d内愈合者18例(36%),对照组2 d内角膜创面愈合者16例(32%),3 d内愈合者34例(68%),两组比较差异有统计学意义(P<0.05),观察组所用时间较短。结论联合角膜绷带镜的综合护理措施可以显著减轻翼状胬肉切除联合自体角膜缘移植术后患者眼部诸多不适症状和疼痛,促进角膜创面愈合。Objective To investigate the effect of bandage contact lens on patients ’ comfort after pterygial excision surgery. Methods One hundred patients with primary unilateral pterygium were randomly divided into control group （non bandage contact lens group） and observation group （bandage contact lens group） after pterygial excision and corneal limbal stem cells transplantation. Patients in control group received traditional nursing and those in observation group wore bandage contact lens in addition to traditional nursing. Patients were surveyed for their comfort after the surgery and the incidences of ocular irritation were compared and corneal wound healing was observed. Results The scores of ocular symptoms such as ocular irritation, burning sensation, irritation, tearing and difficulty in opening eyes in observation group were lower than those in control group （ P〈0.001）. The incidences of ocular pain in observation group was lower than that in control group （P〈0.05）. The time of corneal wound healing in observation group was shorter than that in control group （P〈0.05）. Conclusion Comprehensive nursing interventions combined with bandage contact lens could significantly reduce ocular discomfort and relieve pain of patients with pterygial excision combined with corneal limbal stem cells transplantation, which could promote the progress of corneal wound healing.
