生精方对肾虚型少弱精子症不育患者精子印记基因H19表达的影响

摘　　要：目的观察生精方治疗肾虚型少弱精子症不育患者临床疗效及可能作用机制。方法将67例少弱精子症不育患者随机分为治疗组34例和对照组33例,治疗组给予生精方,每日1剂;对照组给予五子衍宗丸,每次6g,每日2次,共治疗12周。于治疗前后检测患者精子印记基因H19的表达,观察两组患者治疗前后肾虚证候评分、精液常规检查指标变化,观察配偶临床妊娠率及不良反应。结果治疗组治疗后印记基因H19丢失率明显降低(P<0.05);两组患者治疗后肾虚证候评分、精子密度、(a+b)级精子活力均明显改善(P<0.05或P<0.01);治疗后治疗组印记基因H19丢失率、肾虚证候评分、精子密度、(a+b)级精子活力明显优于对照组(P<0.05或P<0.01)。治疗组临床妊娠率为55.88%,对照组为33.33%,治疗组明显优于对照组(P<0.05)。结论生精方可以改善少弱精子症不育患者的精液质量,提高临床妊娠率,其机制可能与改善精子印记基因H19的表达有关。Objective To observe the effects and mechanism of Shengjing Formula （formula to improve the sperm production, 生精方） on infertile patients caused by oligoasthenospermia with kidney deficiency syndrome. Methods Totally 67 cases of infertility caused by oligoasthenospermia were randomized into the treatment group of 34 cases and control group of 33 cases. The treatment group was given Shengjing Formula （生精方） one dose daily. The control group was given Wuzi Yanzong Pills （生精方） 6 g each time, twice daily. The treatment lasted for 3 months. The expression of H19 was detected be- fore and after treatment. The syndrome scores, semen analysis results and adverse reactions were observed. The clini- cal pregnancy rates of spouses were evaluated. Results The loss rate of H19 was significantly improved in the treatment group after treatment （P 〈 0. 05）. The syndrome scores, sperm density and sperm motility （ a ＋ b class） were significantly improved in both groups after treatment （P 〈 0. 05 or P 〈 0.01 ）, especially in the treatment group （P 〈 0.05）. The clinical pregnancy rate in the treatment group and control group was 55.88% and 33.33% respec- tively, with a significant difference between groups （ P 〈 0. 01 ）. Conclusion Shengjing Formula （生精方） can improve semen quality and clinical pregnancy rate possibly by improving the expression of H19 imprinted genes in hu- man sperm.
