白芍总苷对类风湿关节炎患者血脂的影响

摘　　要：目的:观察白芍总苷对类风湿关节炎(rheumatoid arthritis,RA)患者血脂的影响。方法:选取2012年6月至2013年6月我院风湿科门诊和住院的RA患者68例,按随机数字表法分为A、B2组,每组34例。A组联合应用甲氨蝶呤片、来氟米特片治疗,B组在A组治疗基础上加用白芍总苷,分别观察2组RA患者在治疗前及治疗3个月后胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、关节肿胀指数、关节压痛指数、晨僵时间、C-反应蛋白(CRP)、血沉(ESR)、DAS28的变化,同时对2组不良反应情况进行比较。结果:治疗3个月后,2组RA患者DAS28、ESR、CRP、关节压痛数、关节肿胀数和晨僵时间与治疗前比较明显降低,差异有统计学意义(P<0.01)。B组治疗后DAS28、ESR、CPR和晨僵时间改善优于A组,2组比较差异有统计学意义(P<0.05或P<0.01),且B组患者治疗后TC、TG、LDL-C均低于A组,而HDL-C高于A组,经统计学分析差异有统计学意义(P<0.05或P<0.01)。结论:RA患者存在血脂水平异常,白芍总苷治疗类风湿关节炎具有较好的疗效,可明显改善其症状和体征,且对血脂也具有治疗作用。Objective： To observe the effection of total glucoseides of peony on lipid profile in patients with rheumatoid arthritis. Method： From 2012 June to 2013 June in our hospital and outpatient department of rheumatism of 68 RA patients,were randomly divided into A and B two groups,each group of 34 cases. The A group combined with methotrexate and leflunomide for the treatment of RA,B group,based on A group＇s treatment,were treated by total glucoseides of peony.Some index were observed in two group before treatment and 3 months after treatment,cholesterol（ TC）,low density lipoprotein cholesterol（ LDL-C）,triglyceride（ TG）,high density lipoprotein cholesterol（ HDL-C）,swelling index,joint pain index,time of morning stiffness,C-reactive protein（ CRP）,erythrocyte sedimentation rate（ ESR）,DAS28.while the patient in the two group therapy during the occurrence of adverse reactions were statistically analyzed. Results After 3 months of treatment,DAS28,ESR,CRP,joint tenderness,joint swelling and morning stiffness time,both in two groups RA patients,compared with before treatment significantly reduced,there were a very significant difference（ P〈0. 05 or P〈0. 01）. The therapeutic effects of group B of DAS28、ESR、CRP and morning stiffness were better than those of group A（ P〈0. 05 or P〈0. 01） .After 3 months treatment,TC,TG,,LDL-C,in B groups of patients,were lower than that A group,and HDL-C higher than that A group,there were significant differences（ P〈0. 05 or P〈0. 01）.Conclusion： RA patients have abnormal blood lipid levels,total glucoseides of peony has a direct effect on the treatment of rheumatoid arthritis,can effectively improve the symptoms and signs,and also has therapeutic effects on blood lipids.
