摘 要:目的探讨复方氨基酸胶囊对糖尿病肾病患者的临床疗效及对胰岛素抵抗的影响。方法 170例糖尿病肾病患者随机分成观察组和对照组,每组85例。两组均给予贝那普利及常规治疗,观察组在此基础上加用复方氨基酸胶囊治疗,疗程为3个月。比较两组治疗前后临床疗效、胰岛素抵抗指数及药物不良反应的差异。结果①观察组有效率(90.59%)显著高于对照组(75.29%),差异有统计学意义(P<0.05)。②两组治疗后尿白蛋白排泄率(UAER)、胰岛素抵抗指数、空腹血糖、餐后血糖、糖化血红蛋白、收缩压及舒张压均较同组治疗前显著下降,清蛋白较同组治疗前显著上升,差异均有统计学意义(P<0.05);治疗后,观察组UAER、胰岛素抵抗指数、收缩压及舒张压均显著低于对照组,清蛋白高于对照组,差异均有统计学意义(P<0.05)。③观察组总不良反应发生率(15.29%)与对照组(12.94%)比较,差异无统计学意义(P>0.05)。结论复方氨基酸胶囊可能通过改善糖尿病肾病患者胰岛素抵抗来降低UAER,提高临床疗效。Objective To explore the effect of compound amino acid capsules on insulin resistance in the patients with diabetic nephropathy( DN). Methods A total of 170 patients suffered DN were divided into observation group and control group,85 cases in each group. They all accepted the treatment of benazepril and convention treatment,the patients of the observation group were given compound amino acid. The difference of the clinical effect,insulin resistance index and drug adverse reactions were compared between the two group. Results ①The total effective rate in the observed group( 90. 59%) was significantly higher than that in the control group( 75. 29%)( P〈0. 05). ②The urinary albumin excretion rate( UAER),insulin resistance idex,fasting blood glucose,postprandial blood glucose,glycosylated hemoglobin,systolic and diastolic blood pressure significantly declined,and the albumin significantly increased than that before the treatment in both of the groups( P〈0. 05). After treatment,UAER,insulin resistance index,systolic and diastolic blood pressure in the observation group were significantly lower,and the albumin was significantly higher than that in the control group( P〈0. 05). ③There's no significant difference in the rate of drug adverse reactions( 15. 29% vs 12. 94%) between the two groups( P〉0. 05). Conclusion Compound amino acid capsules could improve clinical efficacy for patients with DN by improving insulin resistance and reducing UAER.
