改良Kugel手术治疗复发性腹股沟疝28例

摘　　要：目的：探讨改良 Kugel 手术治疗复发性腹股沟疝的应用价值。方法回顾性分析2012年3月至2013年1月，武警湖北省总队医院收治复发性腹股沟疝患者28例的临床资料，均行改良Kugel手术治疗，观察手术时间、术后恢复自主活动时间、术后住院时间及近、远期并发症发生情况。结果28例患者均顺利完成手术。手术时间45∽72 min，平均（59．6＆#177;9．0）min；术后下床活动时间8∽12 h，平均（9．8＆#177;1．5）h；术后恢复自主活动时间8∽16 h，平均（11．7＆#177;2．6）h；术后住院时间3∽9 d，平均（6．5＆#177;1．3）d。术后并发尿潴留5例，切口疼痛4例，异物感4例，切口及阴囊血肿2例，发热1例。随访10∽21个月无腹股沟疝复发及股疝形成，无感染及慢性疼痛，无射精痛等并发症。结论改良Kugel手术具有微创、无张力、免缝合的特点，能加固薄弱的腹横筋膜层，完整封闭耻骨肌孔，治疗复发性腹股沟疝疗效显著。Objective To investigate the value of modified kugel repair in recurrent inguinal hernia.Methods From March 201 2 to January 201 3,28 cases of recurrent inguinal hernia in Hubei Armed Police Corps Hospital were retrospectively analyzed.Modified kugel repair was performed to all of them.The operating time,the recovery time of returning to activity,length of postoperative hospital stay and the complications in short term and long term,were observed in detail.Results All operations were successfully performed.The mean operation time was （59.6 ＆#177;9.0）minutes,range from 45 to 72 minutes,the mean time to get of bed was （9.8 ＆#177;1 .5）hours,range from 8 to 1 2 hours and time to return to activity was （1 1 .7 ＆#177; 2.6）hours,range from 8 to 1 6 hours,the average length of hospital stay was （6.5 ＆#177;1 .3）days,range from 3 to 9 days.There were 5 cases of urinary retention,4 cases of incision pain,4 cases of foreigen body sensation,2 cases of incision and scrotal hematoma,1 case of fever.With a follow-up of 1 0 to 21 months, there was no recurrence and femoral hernia,and none was complicated with infection and ejaculation pain. Conclusions The modified kugel repair have the characteristics of minimally invasive,no tension and suture-free,and it can also enhance the defect of transversalis fascia and close the myopectineal orifice completely,anyhow,it has remarkable curative effect for inguinal hernia.
