腰椎后路非对称固定经椎间孔椎体间融合的基础及临床研究

摘　　要：目的探讨腰椎后路非对称固定经椎间孔椎体间融合的基础及临床应用效果。方法按照自由选择方式将48例腰椎退行性病变患者分为单侧组(23例)及双侧组(25例)。单侧组给予单侧椎弓根螺钉经椎间孔椎体间融合手术治疗,双侧组给予双侧椎弓根螺钉经椎间孔椎体间融合手术治疗。比较两组的手术情况、治疗效果、预后情况。结果单侧组的手术时间显著短于双侧组,术中出血量显著少于双侧组,治疗费用显著低于双侧组(P<0.05)。两组治疗后的VAS评分、Oswestry评价情况均较治疗前显著改善(P<0.05),两组治疗后的上述指标差异无统计学意义(P>0.05),两组的并发症发生率、融合率比较差异无统计学意义(P>0.05)。结论腰椎后路非对称固定经椎间孔椎体间融合治疗腰椎退行性病变可获得满意的效果及预后,有效缩短手术时间,减少术中出血量,减轻患者的经济负担。Objective To explore the basic and clinical application effect of lumbar posterior asymmetrical fixed trans-foraminal lumbar interbody fusion. Methods According to patients’ will,48 patients with lumbar degenerative disease were divided into the unilateral group （n=23）and the bilateral group （n=25）.The unilateral fixation group was given uni-lateral pedicle screw transforaminal lumbar interbody fusion operation,while the bilateral group was given bilateral uni-lateral pedicle screw transforaminal lumbar interbody fusion operation.The operation situation,curative effect,and prog-nosis of the two groups were compared. Results The operation time in the unilateral group was obviously than that in the bilateral group,intraoperative amount of bleeding in the unilateral group was obviously than that in the bilateral group,the cost of treatment in the unilateral group was obviously lower than that in the bilateral group （P〈0.05）.The score of VAS and Oswestry evaluation after treatment were better than those before treatment in the two groups （P〈0.05）,but there was no statistical difference of above-mentioned indicators between the two groups （P〉0.05）.There was no statistical of fusion rate and incidence rate of complication in the two groups （P〉0.05）. Conclusion Lumbar posteri-or asymmetrical fixed transforaminal lumbar interbody fusion treating lumbar degenerative disease can obtain satisfacto-ry effect and prognosis,and can effectively shorten operation time and reduce the intraoperative amount of bleeding,and ease patients’economic burden.
