摘　　要：目的探讨组织多普勒成像评价急性冠状动脉综合征(ACS)近期预后的价值。方法随机入选79例ACS患者,包括预后良好组(n=58)和预后不良组(n=21)。用超声组织多普勒成像(TDI)检查二尖瓣口舒张早期血流速度峰值(E)与左心室侧壁二尖瓣环舒张早期运动速度(Em)的比值(E/Em)。研究预后不良的ACS患者E/Em的变化。用Pearson相关性分析ACS患者E/Em与血清肌钙蛋白肌钙蛋白I(CTnI)水平的关系。用Logistic回归研究E/Em与血清CTnI异常是否是ACS近期预后不良的独立预测因子。结果与预后良好组比较,预后不良组ACS患者E/Em升高(分别为7.87±1.64、14.26±3.12,P<0.05)。ACS患者E/Em与血清CTnI水平呈正相关,r=0.782,P=0.000。E/Em上调是ACS近期预后不良的独立预测因子(OR=3.12,P=0.041)。结论 E/Em升高是ACS近期预后不良的独立预测因子。Objective To investigate the the predictive value of short-term prognosis in ACS patients with tissue Doppler imaging. Methods 79 patients with acute coronary syndrome（ ACS）,divided into good prognosis group（ n = 58） and poor prognosis group（ n = 21）,were radomly selected. The patients were detected the E /Em ratio of early diastolic mitral inflow peak velocity（ E） to myocardial early diastolic velocities at the lateral mitral annulus（ Em） measured by tissue Doppler imaging. Relation of E /Em ratio and serum CTnIwere analized by using Pearson correlation. Whether abnormal E /Em ratio could be independent predictor of ACS with poor prognosis or not was analized by Logistic regression. Results Compared with good prognosis group,the E /Em ratio increased in poor prognosis group（ 7. 87 ± 1. 64 vs 14. 26 ± 3. 12,P〈0. 05）. Pearson correlation analysis indicted that E /Em ratio was positively associated with the level of serum CTnIin ACS patients（ r= 0. 782,P = 0. 000）. The upregulation of E /Em ratio was independent predictor of ACS with poor prognosis（ OR = 3.12,P =0.041）. Conclusion The upregulation of E/Em ratio has predictive value for ACS with poor prognosis.
