摘 要:目的检测承德地区12岁以下变应性鼻炎(Allergic Rhinitis,AR)伴腺样体肥大(Children Adenoidal Hypertrophy,CAH)患儿血清变应原,并进行结果分析,了解该地区该类患儿变应原分布情况,为临床防治AR提供一定参考。方法回顾性分析69例变应性鼻炎伴腺样体肥大患儿,为观察组;AR不伴有腺样体肥大患儿72例,为对照组。采用酶联免疫吸附法(ELISA),抽取检测两组患儿血清变应原种类及浓度并进行统计学分析,阐述承德地区AR伴有腺样体肥大患儿变应原的分布特点。结果观察组患儿总IgE阳性67例,阳性率97.10%;对照组患儿总IgE阳性70例,阳性率97.22%,P>0.05,二者差异无统计学意义。特异性IgE检测两组均以户尘螨、春季花粉组合、夏秋季花粉组合为主,但观察组阳性率均高于对照组。结论变应原刺激是导致儿童变应性鼻炎和腺样体肥大的一个重要因素,发现变应原并采取有效措施避免与之接触,是预防和治疗变应性鼻炎伴腺样体肥大的关键。承德地区AR伴有腺样体肥大患儿的变应原主要为室外变应原户尘螨、春季花粉组合、夏秋季花粉组合。Objective To detect the serum allergen of children under 12 with allergic rhinitis(allergic rhinitis, AR) accompanied with adenoid hypertrophy(children adenoidal hypertrophy, CAH) in Chengde and analyze the result, in order to know the regional distribution of children with such allergens and to provide clinical prevention and treatment of AR. Methods 69 cases of children with allergic rhinitis accompanied with adenoid hypertrophy were retrospectively analyzed as the observation group, and 72 cases with AR without adenoid hypertrophy as the control group. By using ELISA, types and concentrations of serum allergen in the two groups were detected and statistically analyze to state the distribution characteristics of allergen. Results 67 cases in the observation group were with total IgE-positive, and the positive rate was 97.10%; 70 cases in the control group were with total IgE-positive, the positive rate was 97.22%. There was no statistical difference between the two groups, P0.05. Specific IgE detection showed that the main allergens were household dust mites, pollen in the spring, and pollen in the summer and autumn, but the positive rates were higher in the observation group than those in the control group. Conclusion Allergen stimulation is the leading factor for children allergic rhinitis and adenoid hypertrophy, therefore the key measure is to find allergens and to take effective measures to avoid contact with it. The main allergens for children suffered from AR with adenoid hypertrophy in Chengde are household dust mites, pollen in the spring, and pollen in the summer and autumn.
