摘 要:目的研究急性脑梗死不同性质颈动脉斑块与基质金属蛋白酶-9(matrix metalloproteinase-9,MMP-9)之间的关系,探讨颈动脉粥样硬化斑块稳定性及MMP-9水平在脑梗死发生中的作用及临床意义。方法113例急性脑梗死患者组(ACI)和31例健康对照组,根据颈动脉彩超将脑梗死组分为无斑块组(NP)(39例)、稳定斑块组(SP)(34例)、不稳定斑块组(VP)(40例)3个亚组。测定对照组及急性脑梗死患者发病48 h内和d 14血清MMP-9水平,并进行相关分析。结果急性脑梗死患者入院48 h内和d 14血清MMP-9水平4组比较差异均有统计学意义(P<0.05),血清MMP-9与梗死体积(r=0.289,P=0.002)、空腹血糖(r=0.200,P=0.034)及血压(P<0.05)呈正相关。结论MMP-9参与了动脉粥样硬化的形成及斑块的不稳定性,可能是急性脑梗死的血清标志物。Objective To investigate the relationship between carotid artery plaque of different characteristic and matrix metalloproteinase-9(MMP-9) in patients with acute cerebral infarction, To explore the value of carotid atherosclerotic plaque stability and serum MMP-9 level in acute cerebral infarction. Methods There were 113 patients with acute cerebral infarction(ACl group) and31 cases of healthy persons. All subjects were detected the characteristic by carotid artery ultra-sonography. ACl group was divided into NP, SP and VP. All subjects were measured serum MMP-9 with ELISA within 48 hours of onset and the 14th day, and to analyse the correlation between MMP-9 and carotid artery plaque and acute cerebral infarction. Results The serum levels of MMP-9were significantly higher among the four groups(P〈0.05) within 48 hours of onset and the 14th day in patients with acute cerebral infarction. The serum MMP-9 were related positively to the infarction area(r=0.289, P=0.002), the level of FGB(r=0.200, P=0.034)and the blood pressure(P〈0.05). Conclusion MMP-9 participates in the formation of atherosclerosis and plaque instability, so the serum MMP-9 may be a serum marker of acute cerebral infarction.
