摘  要:目的对比分析江苏盛泽医院重症监护病房(ICU)与非ICU非发酵菌的耐药率,为临床防治多重耐药非发酵菌提供依据,并指导临床合理使用抗生素。方法回顾性分析2011-2013年江苏盛泽医院ICU与非ICU送检细菌标本中检出的非发酵菌对常用抗生素的耐药情况,采用API鉴定系统鉴定细菌;纸片扩散法测定细菌药物敏感性;WHONET 5.6软件进行系统性分析。结果 3年所有送检标本共检出病原菌2 288株,其中非发酵菌906株,占39.6%;分离最多的3种非发酵菌依次为铜绿假单胞菌470株(51.9%)、鲍氏不动杆菌256株(28.3%)、嗜麦芽窄食单胞菌168株(18.5%);在ICU与非ICU耐药率比较中,前述3种非发酵菌对多数常用抗菌药物的耐药率比较差异有统计学意义(P<0.01),且ICU检出菌的耐药率明显高于非ICU。结论该院ICU非发酵菌耐药情况严峻,应合理使用抗生素。Objective To analyze the drug resistance of non-fermenters from ICU vs non-ICU wards in Jiangsu Shengze Hospital so as to provide evidence for the treatment. Methods Non-fermenters isolated in our hospital from 2011 to 2013 were studied retrospectively. The bacteria were identified by API system; The sensitivity to antimicrobial agents were tested by K-B methods; The data was analyzed by WHONETS. 6. Results A total of 2,288 strains of bacteria were detected, including 906 ( 39.6% ) non-fermenters. The top three non-fermenters were Pseudomonas aeruginosa (470, 51.9% ) , Acinetobaeter baumannii (256, 28.3% ) , and Stenotrophomonas maltophilia ( 168, 18.5% ). The drug resistance rate of non-fermenters from ICU wards was significantly higher than those from non-ICU wards ( P 〈 0.01 ). Conclusion The drug resistance of non-fermenters in ICUs is severe. Antibiotics should be used rationally.
