摘 要:目的观察大鼠膀胱出口部分梗阻不同时期的膀胱逼尿肌细胞增生、凋亡及间质中胶原纤维的变化情况。方法 Wistar大鼠40只,采用经会阴途径球部尿道部分结扎的方法建立膀胱出口部分梗阻模型,随机分为假手术组10只,梗阻2周组15只,梗阻4周组15只。采用免疫组化方法分析增殖细胞核抗原(PCNA)表达情况;通过末端转移酶标记(TUNEL)法测逼尿肌细胞凋亡指数;运用VG染色分析胶原纤维占膀胱逼尿肌面积比例变化情况。结果假手术组、梗阻2周组和梗阻4周组PCNA阳性表达率分别为(17.19±1.37)%、(50.74±3.15)%、(33.58±2.80)%,梗阻组之间差异有统计学意义(P<0.05),梗阻组与假手术组之间差异有统计学意义(P<0.05)。假手术组、梗阻2周组和梗阻4周组逼尿肌细胞凋亡指数分别为(20.22±1.70)%、(33.90±1.86)%、(51.18±3.30)%,梗阻组之间差异有统计学意义(P<0.05),梗阻组与假手术组之间差异有统计学意义(P<0.05)。假手术组、梗阻2周组和梗阻4周组胶原纤维占膀胱逼尿肌面积比例分别为(7.07±0.67)%、(13.19±1.50)%、(22.98±2.89)%,梗阻组之间差异有统计学意义(P<0.01),梗阻组与假手术组之间差异有统计学意义(P<0.01)。结论不同时期大鼠膀胱出口梗阻的膀胱逼尿肌形态结构的变化,可能是膀胱逼尿肌细胞增生、凋亡以及间质中胶原纤维占膀胱逼尿肌比例变化的综合作用结果。Objective To observe the proliferation and apoptosis of the bladder detrusor cells in different periods of rat bladder outlet obstruction and the change of interstitial collagen fiber.Methods Bladder outlet obstruction models were es tablished in 40 Wistar rats,which were divided into 3 groups,including the sham-operation group (n =10),2-week obstruction group (n =15) and 4-week obstruction group (n =15).The PCNA expression was analyzed with immunohistochemical method,the apoptotic index was detected with TUNEL,and the changes of collagen fiber in the bladder detrusor were observed with VG staining.Results The expression of PCNA was (17.19-±-1.37)%,(50.74±3.15)% and (33.58±2.80)% in the sham-operation group,2-week obstruction group and 4-week obstruction group,respectively,with significant difference among the groups (P〈0.05).The apoptotic index was (20.22±1.70) %,(33.90±1.86) % and (51.18±3.30)% respectively,with significant difference among the groups (P〈0.05).The proportion of collagen fiber in the bladder detrusor was (7.07± 0.67) %,(13.19 ± 1.50) %,and (22.98 ± 2.89) %,respectively,with significant difference among the groups (P 〈 0.01).Conclusiorns Changes of shape and structure of bladder detrusor in different periods of bladder outlet obstruction may be caused by cell proliferation,apoptosis,and changing proportion of interstitial collagen fiber.
