摘  要:目的:利用3.0 T磁共振成像技术(MRI)评价扩张型心肌病(DCM)患者的左心结构、功能特征及其对DCM的诊断价值。方法:对45例DCM 患者及30例健康者对照进行 MRI检查,测得左心房及左心室的收缩末期容积、舒张末期容积、收缩末期内径及舒张末期内径,左心室收缩末期及舒张末期球形指数。结果:DCM患者左心室射血分数小于健康对照,其余所有左心结构指标及功能参数都大于健康对照(P<0.05);左心结构与功能参数相关性好,且所有参数诊断DCM的受试者工作特征曲线(ROC)下面积在0.850-1.000之间,当射血分数阈值取33.1%时,敏感度和特异度可达100%。结论:MRI可评价DCM患者左心结构及功能改变,对DCM有很好的诊断价值。Objective:To assess the correlation between the morphologic and functional parameters of left heart using cardiac magnetic resonance imaging (MRI)and its application on dilated cardiomyopathy (DCM).Methods:Forty-five patients with DCM and 30 normal subjects underwent cardiac MRI.The volumes,dimensions and sphericity of the left ventricle and atrium in end systolic and end diastolic phase,and ejection fraction were measured.Results:Except that the ejection fraction of DCM were significantly lower than controls,the other parameters of left ventricle and left atrium were significantly higher than control subjects (P〈0.05).There were good correlation between morphologic and functional parameters of left heart,and the parameters might predict DCM (area under receiver operating characteristic curve ROC= 0.850-1.000,P〈0.05).Sensitivity and specificity of 100% were achieved for differentiating DCM and controls by using the ejection fraction′s cutoff value of 33.1%.Conclusion:MRI can evaluate the morphological and functional changes of DCM compared with the controls,which can be an indicator for diagnosing DCM.
