摘 要:目的调查2012—2013年广州市散发性腹泻诺如病毒(NoV)流行状况和流行特点。方法收集2012—2013年广州地区门急诊散发性腹泻患者粪便标本2236份,采用荧光定量RT—PCR对采集的标本进行NoV核酸检测并初步分型,收集并分析病人的临床基本信息,分析NoV阳性检出率在不同年龄段、性别、月份季节差异,同时对NoV阳性的标本进行A组轮状病毒、B组轮状病毒、C组轮状病毒、腺病毒、札如病毒、星状病毒检测,研究其混合感染情况。结果NoV阳性检出率为10.60%(237/2236),以GⅡ型为主。NoV在18~40岁的青年人中检出率最高(16.47%),7~17岁少年最低(3.67%),男女检出率差异无统计学意义(P〉0.05)。NoV全年都可检测到,夏季为高发季节,其中7-8月份检出率最高。NoV与其他肠道病毒共感染率为14.35%(34/237),其中与A组轮状病毒共感染率最高。结论广州地区NoV夏季为高发季节。在18-40岁的青年人中检出率最高,以GⅡ型感染为主,混合感染最常见为A组轮状病毒。Objective To investigate the prevalence status and characteristics of norovirus infection among sporadic cases of diarrhea in Guangzhou area from 2012 to 2013. Methods A total of 2 236 diarrhea outpatients' feces were collected from 2012 to 2013 in Guangzhou area. The DNA of NoV and preliminary genotype were detected by RT-PCR. Clinical information of the patients was collected and analyzed for the differences of NoV positive detection rates in different ages, gender, month and season. For the NoV positive samples, another six enterovirus including group A rotavirus (GARV), adenoviruses (AdV) were detected to analyze the co-infection conditions. Results The positive detection rate of NoV was 10.60%. GII was the most common genotype. The people of 18-40 years-old had the highest positive detection rate, while 7-17 years-old had the lowest. There was no statistical significant difference in the infection rate between males and females. The detection rate of NoV in summer was significantly higher than other seasons, while July and August had the highest detection rates. The co-infection rate was 14.35% and GARV was the most common co-infection virus. Conclusion The detection rate of NoV in summer was the highest in Guangzhou area. 18-40 years-old young people had the highest positive detection rate. GII was the most common genotype. GARV was the most likely co-infection viruses.
