摘 要:目的探讨人血浆在金黄色葡萄球菌血浆凝固酶试验中的应用效果及可行性。方法用人血浆、兔血浆、冻干血浆进行血浆凝固酶试验比对,以及用基层实验室易获得的不同来源人血浆、不同抗凝剂人血浆、不同保存期人血浆进行血浆凝固酶试验分析。结果人血浆和兔血浆血浆凝固酶试验结果一致,阳性率均为100.0%。冻干血浆试验平均阳性率为40.6%,冻干血浆与人血浆、兔血浆试验阳性率比较差异有统计学意义(χ2=27.022,P<0.05)。从业人员、学生、门诊患者血浆试验阳性率分别为93.8%、96.9%、78.1%,差异有统计学意义(χ2=6.753,P<0.05)。EDTA-K2、肝素钠、枸橼酸钠抗凝人血浆试验阳性率分别为96.9%、100.0%、96.9%,差异无统计学意义(χ2=2.065,P>0.05)。新鲜、1周、2周、4周保存期人血浆试验阳性率分别为96.9%、93.8%、100.0%、90.6%,差异无统计学意义(χ2=3.497,P>0.05),8周、12周保存期的人血浆试验阳性率分别为71.8%、59.4%,试验阳性率明显下降。结论基层实验室检测金黄色葡萄球菌可以用人血浆代替兔血浆做血浆凝固酶试验。Objective To explore the application effect of human plasma in Staphylococcus aureus coagulase test and feasibility. Methods Human plasma, rabbit plasma, freeze-dried plasma in the coagulase test, different sources of human plasma that could be easily obtained by basic laboratories, and different anticoagulant plasma, different storage period of human plasma were used in the coagulase test. Results The positive rates of human plasma and rabbit plasma test were both 100.0%, consistent with the results. The positive rate of freeze-dried plasma was 40.6% on average. The positive rates of freeze- dried plasma and human plasma, and rabbit plasma were significantly different (X^2=27.022, P〈0.05). The positive rates of practitioners, students, and outpatients were 93.8%, 96.9%, and 78.1%, respectively, with statistical significant differences (x^2=6.753,P〈0.05). The positive rates of samples with EDTA-K:, heparin, or sodium citrate were 96.9%, 100.0%, and 96.9%, respectively, showing no significant differences. The positive rates of fresh, 1 week, 2 weeks, and 4 weeks preserved human blood plasma s were 96.9%, 93.8%, 100.0%, and 90.6%, respectively, showing no significant differences. The positive rates of blood stored for 8 weeks, 12 weeks were 71.8% and 59.4%, respectively, showing significant decrease. Conclusion Detection of Staphylococcus aureus in basic laboratories can use human plasma instead of rabbit plasma in the coagulase test.
