颈淋巴结结核患者外周血和组织中T-bet表达的检测

摘　　要：目的探讨颈淋巴结结核患者外周血和组织中T-box基因家族转录因子(T-bet)的表达,并分析在结核病发病中的意义。方法采用实时荧光定量PCR(Realtime-PCR)检测了12例颈淋巴结结核患者和11例健康对照外周血T-bet的表达情况,使用GraphPad Prism 4.0统计软件进行分析,两组间比较采用配对t检验,以P<0.05为差异有统计学意义;同时采用免疫组织化学双染法检测颈淋巴结结核患者淋巴结组织中T-bet的表达情况,计数每个高倍视野中CD4+细胞和CD20+B细胞T-bet阳性细胞的百分率。结果在颈淋巴结结核患者外周血PBMC中T-bet的表达水平为6.07±3.35显著低于健康对照20.12±11.31(t=4.118,P=0.0005);淋巴结核结节中,CD4+细胞T-bet阳性率为65.7%,CD20+B细胞T-bet阳性率为5.0%。结论 T-bet在颈淋巴结结核外周血中低表达,而在淋巴组织中高表达,提示T-bet在颈淋巴结结核免疫发病中起重要的作用。Objective To detect the expression of T-bet in blood and tissue from patients with cervical lymph node tuberculosis and its clinical siginificance.Methods The mRNA level of T-bet in blood from 12 patients with cervical lymph node tuberculosis and 11 healthy controls were detected by realtime-PCR and analyzed using GraphPad Prism 4.0 software.At the same time,the expression of T- bet in CD4~＋ cells and CD20~＋B cell in the tissue of lymph node were detected by immunohistochemical double staining.Result The mRNA level of T-bet in PBMC from cervical lymph node of TB patients（6.07±3.35） was much lower than those in healthy control（20.12±11.31）（t= 4.118,P=0.0005）.In the lymph node,the positive rate of T-bet CD4~＋ cells was about 65.7%and 5.0%in the CD20~＋ B cells.Conclusion The expression of T-bet was low in blood from the patients with cervical lymph node tuberculosis.And T-bet was highly expressed in the lymph node.Therefore,T-bet plays an important role in the occurrence of cervical lymph node tuberculosis.
