摘 要:目的探索玉溪市伤寒/副伤寒流行危险因素,为制定针对性控制方案提供科学依据。方法对玉溪市700例伤寒/副伤寒病例按照1:1配比,采用病例对照方法进行研究分析。结果不同流行强度区域,差异有统计学意义的危险因素各不相同,高流行区为经常在外就餐(χ2=8.28,P﹪0.05 OR=1.68)、经常吃凉菜(χ2=6.78,P﹪0.05)和经常吃烧烤(χ2=4.27,P﹪0.05),中流行区为经常饮用生水(χ2=21.99,P﹪0.05)、经常饮井水(χ2=17.35,P﹪0.05),低流行区为有接触史(χ2=10.06,P﹪0.05)、经常饮用生水(χ2=11.81,P﹪0.05)和经常饮井水(χ2=10.06,P﹪0.05)。结论玉溪市伤寒/副伤寒不同流行区域主要流行危险因素不同,高流行区流行危险因素是经常在外就餐、经常吃凉菜和经常吃烧烤,中流行区的流行危险因素是经常饮用生水、井水,低流行区的危险因素是接触史、经常饮用生水、井水。因此根据不同区域,采取差异化防控措施是必要和可行的。Objective To understand the risk factors associated with prevalence of typhoid/paratyphoid in Yuxi city.Methods The 1:1 match case-control method was used to analyze 700 typhoid/paratyphoid cases in Yuxi.city.Results Significant risk factors associated with prevalence of typhoid and paratyphoid were noticed among areas with different prevalent intensity.In high prelent intensity areas,the rick factors were often eating outside(χ^2=8.28,P 0.05,OR=1.68),often eating cold dish(χ^2=6.78,P 0.05) and often barbecuing(χ^2=4.27,P 0.05).In moderate prevalent intensity areas,the risk factors were often drinking unboiled water(χ^2=21.99,P 0.05) and well water(χ^2=17.35,P 0.05).In low areas,the risk factors were history of contact with the diseases(χ^2=10.06,P 0.05),often drinking unboiled water(χ^2=11.81,P 0.05) and well water(χ^2=10.06,P 0.05).Conclusion There were different risk factors associated with the prevalence of typhoid and paratyphoid in areas with different prevalent intensity in Yuxi.The risk factors were often eating in the outside,often eating cold dish and often barbecuing in high prevalent intensity areas,and often drinking unboiled water and well water were risk factors in moderate intensity areas,while in low areas,the risk factors were history of contact with the diseases,often drinking unboiled water and well water.Thus concrete control measures be taken in prevalent intensity areas.
