摘　　要：目的:系统评价福辛普利治疗心力衰竭的有效性。方法:计算机检索Cochrane图书馆、PubMed、Embase、Medline、CNKI、VIP、万方数据库等,检索时间均从建库至2013年10月31日,查找所有关于福辛普利治疗心力衰竭的随机对照试验的中、英文文献。由两名系统评价员独立进行资料提取并评价纳入研究的文献质量。对纳入的随机对照试验文献采用RevMan 5.2软件进行Meta分析。结果:共检索到符合条件的文献796篇,符合纳入标准的RCT文献19篇(n=2 623),纳入研究文献多数方法学质量为中等。总有效率、左心室功能、6 min步行距离等方面的Meta分析结果显示,福辛普利疗效优于安慰剂及常规治疗药物,福辛普利联合用药优于单用福辛普利。结论:在治疗心力衰竭方面,福辛普利相对于安慰剂及常规治疗药物具有明显的治疗作用。此外,福辛普利结合常规药物治疗心力衰竭,不仅能缓解患者临床症状、提高运动耐量、改善预后和生活质量,而且服药方便、依从性好、不良反应少,在治疗心力衰竭方面有很好的临床应用前景。OBJECTIVE：To systematically evaluate the clinical efficacy of fosinopril for heart failure.METHODS：The literature （either in English or Chinese） of the randomized clinical trials （RCTs） on fosinopril for treatment of heart failure were retrieved from the Cochrane Library,PubMed,Embase,Medline and CNKI,VIP and Wanfang databases from database initiation to Oct.31,2013.Two reviewers independently retrieved the data and evaluated the quality of the included studies.The RCTs included were analyzed using RevMan 5.2 Meta-analysis software.RESULTS：A total of 796 papers meeting the criteria were retrieved.Nineteen RCTs involving 2 623 patients met our inclusion criteria.Most of the included studies were evaluated as middle-level in methodological quality.Meta-analyses on total effective rate,left ventricular function and walking distance in 6 minutes （6MTH）showed that fosinopril was better than placebo and conventional therapy and fosinopril used in combination with other drugs was better than fosinopril alone in efficacy for heart failure.CONCLUSIONS：Fosinopril has a remarkable therapeutic effect for heart failure compared to placebo or conventional therapy.In addition,fosinopril used in combination with conventional medications for heart failure can alleviate patients＇ clinical symptoms,raise exercise tolerance,improve prognosis and quality of life; meanwhile,it is convenient to use with few adverse reaction and to which patients had good tolerance,thus posing promising clinical application prospects in the treatment of heart failure.
