35例高龄前列腺增生症患者经尿道前列腺汽化电切术的治疗体会

摘　　要：目的 探讨高龄前列腺增生患者（BPH）应用经尿道前列腺汽化电切术（TUVP）治疗的疗效及安全性.方法 回顾性分析2010年1月至2013年12月在我院年龄≥90岁的35例前列腺增生患者行经尿道前列腺电切治疗,并对术中、术后安全性、术后疗效、并发症进行评估.结果 35例患者均成功行经尿道前列腺汽化电切术,其中30例术后顺利出院,随访3～6个月,术后排尿通畅,无明显并发症.术后2例出血较多：1例再次手术止血;1例给予充盈导尿管气囊至60 ml,压迫前列腺窝止血成功.1例术后第7d出现左下肢深静脉血栓,给予溶栓治疗后顺利出院.1例术后第3d出现发热,考虑双下肺肺炎,给予对症治疗后术后10 d出院.1例术后拔除导尿管后出现轻度尿失禁,给予生物反馈治疗后术后2个月尿失禁基本恢复.结论 经尿道前列腺汽化电切术治疗高龄前列腺增生（BPH）具有疗效好、安全性好等优点,适用于高龄前列腺增生患者.Objective To investigate the efficacy and safety of transurethral vaporization resection of the prostate （TUVP） in treating elderly patients with benign prostatic hyperplasia （BPH）.Methods Retrospective analysis was carried out to 35 elderly patients with BPH aged ≥ 90 years treated by TUVP in our hospital from January 2010 to December 2013.Surgery safety,postoperative safety,postoperative complications and postoperative complications were assessed.Results 35 patients were successfully implemented with TUVP,including 30 cases successfully discharged and urination,no significant complications following up for 3-6 months.2 cases with postoperative bleeding more：1 case given surgery to stop bleeding again; another was treated with filling catheter balloon to 60 ml,oppression prostate fossa hemostasis.1 case occurred left lower extremity deep vein thrombosis after the seventh days,discharged after thrombolytic therapy.One fever patient appeared after the third day,considered as double lung pneumonia,discharged after being given symptomatic treatment for 10 days.1 case occurred mild urinary incontinence after postoperative catheter removal,given biofeedback treatment,urinary incontinence basically recovered 2 months later.Conclusion TUVP is a safe and effective way to deal with the senior patients with BPH.
