宫颈韧带钳夹术在预防产后出血中的临床作用

摘　　要：目的 研究中弯止血钳钳夹两侧宫颈韧带在预防产后出血中的临床作用.方法 利用随机数法将2011年3月至2013年10月在本院产科经阴道顺产的327例产妇随机分为两组,其中钳夹组164例于胎盘娩出后立即用中弯止血钳于宫颈3点与9点两位置钳夹产妇宫颈两侧韧带2小时;对照组163例胎盘娩出后不钳夹产妇宫颈两侧韧带.分别计算两组产妇产时及产后2、24小时3个时间点的出血量,同时观察中弯止血钳钳夹宫颈两侧韧带后软产道及宫颈裂伤等副损伤情况.结果 钳夹组与对照组产妇产时出血量分别为（137.71±72.35）ml和（138.24±73.01）ml,两组比较差异无统计学意义（P＞ 0.05）;两组产妇产后2、24小时出血量比较,钳夹组出血量均低于对照组,差异有统计学意义（P＜0.05）.两组产妇产后软产道及宫颈裂伤发生率比较,差异无统计学意义（P＞ 0.05）.结论 中弯止血钳钳夹宫颈两侧韧带能有效减少产后2、24小时出血量,在预防产后出血中发挥了一定的作用,有可能成为一项预防产后出血的适宜技术.Objective To study the clinical efficacy of clamping bilateral transverse cervical ligaments in the prevention of postpartum hemorrhage.Methods 327 parturients who underwent vaginal delivery in our hospital from March 2011 to October 2013 were randomly divided into two groups by means of random number table.For 164 parturients in clamping group,bilateral transverse cervical ligaments were clamped by curved clamps for 2 hours once the placenta was delivered,while no clamping for 163 parturients in control group.Compared blood loss during the delivery,2 and 24 hours after the delivery,and the injury caused by curved clamps in the clamping of bilateral transverse cervical ligaments such as laceration of cervix and soft birth canal.Results The blood loss in clamping group and control group were （137.71 ± 72.35） ml and （138.24 ± 73.01） ml respectively,without statistically significant difference between two groups （P ＞ 0.05）.The blood loss 2 and 24 hours after the delivery in the clamping group were less than those in control group,with statistically significant differences （P ＜ 0.05）.There was no statistically significant difference in incidence of laceration of cervix and soft birth canal between two groups （P ＞ 0.05）.Conclusion Clamping bilateral transverse cervical ligaments by curved clamps can significantly decrease postpartum hemorrhage,which could be a good technique for prevention of postpartum hemorrhage.
