肾功能正常患者碘对比剂静脉泌尿系造影和CT增强扫描后尿液NGAL结果分析

摘　　要：目的观察肾功能正常患者作碘对比剂静脉泌尿系造影和CT增强扫描后尿液中性粒细胞明胶酶相关脂质运载蛋白(neutrophil gelatinase-associated lipocalin,NGAL)的变化。方法选择127例肾功能正常作碘对比剂静脉泌尿系造影和CT增强扫描的患者,检测其碘对比剂推注前血浆肌酐(Scr)和尿液NGAL水平,碘对比剂推注后4 h、12 h、24 h尿液NGAL及12 h、24 h、48 h、72 h的Scr水平。结果 1例患者诊断为对比剂肾病(CIN),其尿液NGAL在碘对比剂推注后4 h明显升高,比Scr升高早48 h以上。结论 NGAL是CIN早期、敏感的诊断指标。Objective To observe the changes of urine neutrophil gelatinase-associated lipocalin（NGAL）in patients with normal renal function after urinary iodine contrast agent vein angiography and CT enhanced scan. Methods One hundred and twenty-seven patients with normal renal function underwent urinary iodine contrast agent vein angiography and CT enhanced scan were selected. The serum creatinine（Scr）and urine NGAL levels before iodine contrast agent injection,and the urine NGAL levels at 4 h,12 h,24 h after iodine contrast agent injection,the Scr levels at 12 h,24 h,48 h,72 h after iodine contrast agent injection were detected. Results One case was diagnosed as contrast medium nephropathy（CIN）,the urine NGAL level was elevated obviously at 4 hours after iodine contrast agent injection, which is 48 h earlier than the elevation of Scr. Conclusion NGAL is an early and sensitive diagnosis indicators of CIN.
