摘 要:目的观察海岛模拟湿热环境下脊髓火器伤后脊髓组织的病理形态学变化,分析其损伤机制与原因。方法选用南方实验用猪构建湿热环境下脊髓火器伤模型,于6 h后取脊髓组织观察其大体形态变化及病理形态学改变。结果脊髓连续性遭到严重破坏,发生碎裂、坏死、缺损,电镜下可见脊髓硬脊膜血管急剧扩张,硬膜下广泛瘀血,灰质中神经元细胞变性等严重病理改变。结论直接致伤作用、瞬时空腔效应、压力波致伤效应和血液扰动效应等四种机制使脊髓受到严重损伤,而湿热环境下这种损伤更为严重。Objective To observe the pathomorphological changes of the spinal cord after spinal cord firearm injury in the environment of high temperature and humidity and analyse the mechanism and cause of injury. Methods Gunshot models with spinal cord injury were established using southern healthy porcine. Spinal cord changes were observed and optical microscopy(OM) was performed after six hours of injury. Results Continuity of the spinal cord was destructed,and there was fragmentation, necrosis and defect. Under OM, the blood vessels of spinal dura vascular rapidly expanded. Subdural blood congestion was obvious and neurons in grey matter were degenerated. Conclusion The spinal cord could be damaged by four kinds of mechanisms, including direct injury, instantaneous cavity effect, pressure wave injury effect and blood disturbance , which cause more serious injury under hot and humid environment.
