穿刺体位对剖宫产腰硬联合麻醉效果的影响

摘　　要：目的探讨体位对剖宫产腰硬联合麻醉效果的影响。方法潍坊医学院附属医院择期剖宫产100例，随机分为左侧卧组（L组）及右侧卧组（R组），各50例，L2-3穿刺，重比重罗哌卡因15mg腰麻，调节上至平面至T6-8，观察肌肉松弛程度，术中疼痛程度为有效性比较，记录血压下降程度、恶心呕吐及呼吸窘迫感等副作用为安全性比较。结果L组较R组肌肉松弛程度好，镇痛效果佳，血压下降例数、恶心呕吐及呼吸窘迫感均少。结论剖宫产硬腰联合麻醉左侧卧穿刺较右侧卧位更安全有效。Objective To discuss the position effect of CSEA for Cesarean section. Methods 100 cases undergoing elective Cesarean Section in the affiliated hospital of Weifang Medical Univesity were randomly divided into the left side group （group L） and the right side group （group R ）, each 50 cases, all cases punctured in L2-3,injected the heavy proportion of ropivacaine 15mg,and regulated the block level of T6-8,Recorded the degree of muscle relaxation,intraoperative pain degree for comparative effectiveness, vomiting and respiratory distress,measured the blood pressure. Results The muscle relaxation and analgesia of group L was better than group R,the drop of BP,vomiting and respiratory distress were less. Conclusion Left side Position of Combined Spinal and epidural anesthesia for Cesarean Section is more safe and effective than right side.
