胃炎口服液联合PPI三联疗法治疗幽门螺杆菌性消化性溃疡临床研究

摘　　要：目的探讨胃炎口服液联合常规PPI三联疗法治疗幽门螺杆菌(Hp)相关性消化性溃疡的临床疗效及安全性。方法 110例Hp阳性消化性溃疡患者随机分为治疗组和对照组,治疗组在PPI三联疗法的基础上联用中药胃炎口服液治疗,对照组给予PPI三联疗法治疗。记录治疗前后临床症状积分;停药4w后复查胃镜和14 C呼气试验,判断溃疡愈合率和Hp根除率;停药1年后复查14 C呼气试验,判断Hp复发率。观察治疗期间的药物不良反应。结果对照组胃脘疼痛、烧心泛酸、口苦口臭、大便稀溏改善明显(P<0.01),其它症状无明显改善;治疗组各主次症状改善明显,治疗前后临床症状积分显著下降(P<0.01)。治疗组胃镜溃疡疗效总有效率为92.7%,对照组为70.9%;治疗组Hp抑杀率为85.5%,对照组为63.6%;治疗1年后,治疗组复发率为10.9%,对照组为27.3%;治疗组不良反应发生率为14.5%,对照组为43.6%。上述各指标两组比较,差异有统计学意义(P<0.05)。结论中药胃炎口服液联合PPI三联疗法能有效根除Hp相关性消化性溃疡,提高临床疗效,复发率低,安全有效。Objective To evaluate the effect of the weiyan oral liquid combined with PPI triple regimen on the peptic ulcer caused by Helicobacter Pylori （Hp）. Methods 110 patients with Hp-related peptic ulcer were randomly divided into treatment and control groups： the treatment group was treated by the weiyan oral liquid combined with PPI triple regimen for Hp eradication for 1 week,the control group received standard triple anti -Hp therapy for 1 week. Symptom scores were recorded before and after treatment; the endoscopy and H C breath test were used to determine the rate of ulcer healing and Hp eradication 4 weeks after drug withdrawal. Hp recurrence rate was determined by ~4 C breath test 1 year after drug withdrawal. Adverse drug reactions were monitored during the treatment. Results Primary and secondary symptoms of the treatment group were improved significantly after treatment,clinical symptom scores were significantly decreased （P〈0.01）. Epigastric pain,heartburn bitter taste of mouth, and halitosis were improved significantly （P〈0.01）. However, no obvious improvement of other symptoms was observed. The ulcer healing rate of treatment group and control group was 92.7 % and 70.9 % respectively, the difference between two groups was statistically significant （P〈0.05）. The Hp eradication rate of treatment group and control group was 85.5% and 63.6% respectively, the difference between two groups was statistically significant （P〈0. 05）. The Hp recurrence rate of treatment group and control group was 10. 9% and 27. 3% respectively,difference between two groups was statistically significant （P〈0.01）. Conclusion The weiyan oral liquid combined with PPI triple regimen treatment on the Hp-related peptic ulcer can effectively eradicate Hp, increase the clinical efficacy and reduce the Hp recurrence rate.
