摘 要:目的:探讨糖尿病合并慢性心衰病人的糖化血红蛋白(GHbA1c)和BNP(BNP)检测的临床意义。方法:医院收治的138例病人,实验组为糖尿病合并慢性患者63例,对照组为糖尿病未并发心衰的患者75例。通过注射甘精胰岛素和口服瑞格列奈,对患者进行14d的治疗,在第0、7、14天时分别检测其空腹血糖(FBG)、GHbA1c、BNP,并对GHbA1c、BNP的相关性进行分析。结果:通过14d治疗,两组患者的FBG和GHbA1c水平显著降低(P<0.05),实验组BNP水平显著降低(P<0.05),但对照组并未有显著影响。在第0、7、14天时,实验组GHbA1c、BNP相关性分别为(r=0.34,P<0.05;r=0.11,P<0.05;r=0.5,P<0.05),.相关性由强变弱,逐渐降低。而对照组在第0、7、14天时,GHbA1c、BNP相关性分比为(r=0.34,P<0.05;r=0.28,P<0.05;r=0.36,P<0.05),均呈现弱相关。结论:糖尿病合并慢性心衰的患者中,随着GHbA1c水平的增加,BNP水平会相应增加。通过控制GHbA1c的水平,可相应降低BNP水平,从而降低慢性心衰患者住院的几率。Objective: To discuss the clinical significance of glycated hemoglobin and brain natriuretic peptide detection in diabetics with chronic heart failure. Methods: 138 patients in the First Hospital of Yangtze University had been divided into the experimental group with 63 patients suffered from both diabetes and chronic heart failure and the control group with 75 diabetics without chronic heart failure. All the patients had been treated by injection of insulin glargine and oral repaglinide for 14 days. On the 0th,7th,and 14 th days,fasting blood glucose( FBG),GHBA1 C and BNP levels had been detected and correlation analysis between GHBA1 C and BNP had been carried out. Results: After 14 days treatment,The levels of FBG and GHbA1 c in the two groups were significantly lower( P < 0. 05). The BNP level was significantly lower in the experimental group( P <0.05). However,there was no significant change in the control group. On the 0th,7th,14 th days the GHBA1 C,BNP correlation coefficients in the experimental group were( r = 0. 34,P < 0. 05; r = 0. 11,P < 0. 05; r = 0. 5,P < 0. 05). The correlation between GHbA1 c and BNP decreased gradually. The correlation coefficients in the control group were( r = 0. 34,P <0. 05; r = 0. 28,P < 0. 05; r = 0. 36,P < 0. 05),which showed a weak correlation. Conclusion: As for those diabetic patients with chronic heart failure,BNP levels will increase accordingly with the GHBA1 C level increasing,and BNP levels may be reduced accordingly by controlling the level of GHBA1 C. Thereby the probability of hospitalization of diabetics with chronic heart failure can be reduced.
