导丝引导支撑插管在先天性心脏病合并尿道下裂中的应用研究

摘　　要：目的：探讨中心静脉导丝引导支撑硅胶尿管插管在先天性心脏病合并尿道下裂中的应用可行性与价值。方法选取我科2011年3月至2013年8月期间手术的22例先天性心脏病合并尿道下裂患儿为研究对象，采用随机数字法分成观察组和对照组各11例，观察组应用中心静脉导丝引导支撑硅胶尿管插管，对照组采用常规插尿管方法进行插管，比较两组插管成功率、插管时间、插管后尿道口水肿、出血例数，术中、术后引流通.情况，及拔管后排尿情况。结果两组插管成功率、插管时间、插管后尿道口水肿例数、拔管后排尿情况差异有统计学意义（ P＜0.05）；而插管后尿道口出血例数，术中、术后引流通.情况差异无统计学意义（ P＞0.05）。结论中心静脉导丝引导支撑硅胶尿管插管应用于先天性心脏病合并尿道下裂患儿，明显优于常规插尿管方法，从而有效解决先天性心脏病合并尿道下裂患者的插尿管困难的问题。Objective To Investigate the application and value of the central venous guide wire silicon urinary catheter in congeni -tal heart disease with hypospadias .Methods A study was performed among 22 hypospadias patients who were divided into two groups with different kinds of test methods from March 2011 to August 2013.11 of them were treated with guide wire silicon urinary catheter , whereas oth-er applied a conventional method , comparison of two groups of the success rate of intubation , intubation time , the number of cases of ure-thral mouth edema , hemorrhage after intubation , patency drainage , postoperative voiding after extubation .Results Two groups of the suc-cess rate of intubation , intubation time , intubation after urethral mouth edema cases and extubation after voiding had significant differences （ P〈0.05 ）;After intubation urethral mouth bleeding , intraoperative and postoperative drainage were no significant differences （ P〉0.05 ） . Conclusions The central venous guide wire silicon urinary catheter in congenital heart disease with hypospadias are superior to the conven -tional method of inserting catheter , and it is able to solve the trouble in inserting tubes into the genitals of the patients with congenital heart disease and hypospadias .
