关节镜下Endobutton钢板内固定治疗肩锁关节脱位的疗效

摘　　要：目的 观察和分析关节镜下采用Endobutton钢板内固定治疗肩锁关节脱位的方法及临床疗效.方法 回顾性分析2011年1月至2013年2月本科72例RockwoodⅢ型肩锁关节脱位手术治疗患者的临床资料.其中切开复位锁骨钩内固定31例为对照组,关节镜下Endobutton钢板内固定治疗41例为观察组.比较两组手术方法及临床疗效.结果 两组患者手术时间及术中出血差异无统计学意义,观察组Karlsson评分肩关节功能恢复总优良率方面高于对照组（97.5％比77.4％,P＜0.05）;在肩部疼痛、肌力减退、关节活动受限等术后并发症发生率低于对照组（均P＜0.05）.结论 关节镜下采用Endobutton钢板内固定治疗肩锁关节脱位临床疗效显著,术后肩部疼痛较传统手术发生率低.Objective To observe the efficacy of Endobutton plate fixation by arthroscope on treating acromioclavicular dislocation.Methods A retrospective analysis was conducted in 72 Rockwood Ⅲ acromioclavicular joint dislocation surgery patients from January 2011 to February 2013.Thirty-one cases of opening reduction and internal fixation of clavicular hook were selected as control group,41 cases of Endobutton plate fixation by arthroscope were selected as observation group.Surgical methods and clinical efficacy were compared between two groups.Results There were no significant differences of operative time and blood loss between groups.Karlsson score of overall good shoulder function recovery rate was higher in observation group （97.5％ vs 77.4％,P＜0.05）.Observation group also showed lower incidences of shoulder pain,decreased muscle strength,joint mobility and other postoperative when compared to the control group （P＜0.05）.Conclusion Arthroscopic Endobutton plate fixation of the acromioclavicular joint dislocation shows better efficacy,with lower incidence of shoulder pain after surgery compared to traditional surgery.
