防旋型股骨近端髓内钉与Gamma钉治疗老年股骨转子间骨折的疗效观察

摘　　要：目的对比分析防旋型股骨近端髓内钉(PFNA)与Gamma钉用于治疗老年股骨转子间骨折的临床疗效。方法收集老年股骨转子间骨折患者99例,53例患者以PFNA钉治疗,另46例患者以Gamma钉治疗。结果 PFNA组的手术时间以及术中出血量均显著少于Gamma组,PFNA组的并发症发生率显著低于Gamma组,P均<0.05;二组骨折愈合时间以及治疗优良率差异无统计学意义,P>0.05。结论PFNA钉与Gamma钉固定治疗老年股骨转子间骨折固定方法可靠、疗效显著,虽均为治疗老年股骨转子间骨折的可靠方法,但PFNA固定创伤小、出血量更少,同时并发症较少,更利于患者早日康复,是更加理想的治疗方法。Objective To comparatively analyze the effect of anti- rotation proximal femoral nail （PFNA） and Gamma nail on the treatment of femoral intertrochanteric fractures in the elderly. Methods Totally 99 cases of elderly patients with intertrochanteric fractures were collected, 53 patients with the treatment of PFNA nail, the other 46 Patients with the treatment of Gamma nail. Results The amount of bleeding in operation time and intraoperative time in the PFNA group was significantly less than the Gamma group, the incidence of complications in the PFNA group was lower than that in the Gamma group, P〈0.05 group. The fracture healing time and the rat of good treatment in the two groups had no significant difference, P〉0.05. Conclusion The effects of both of the PFNA nail fixation and the Gamma nail fixation which are the treatment method of intertrochanteric fracture in elderly patients are reliable and obvious, but the PFNA fixed has small trauma, less bleeding, fewer complications, and is more conducive to recovery of the early patients, and is a more ideal treatment method.
